The Utility of Barium Swallow Studies for Evaluation of Pharyngocutaneous Fistula After Total Laryngectomy

Abstract: Objective: The purpose of this study is to determine the utility of radiographic swallow study following a total laryngectomy or total laryngopharyngectomy when evaluating for presence of pharyngocutaneous fistula. Methods: We performed a retrospective analysis of patients undergoing total laryngectomy and total laryngopharyngectomy at University of Maryland Medical Center between the years of 2009-2014. Patients underwent closure of surgical defect by a variety of methods including primary closure, regional m… Show more

“…Those patients with radiographic findings suggestive of an anastomotic leak, but without clinical evidence of PCF, were included in the non-PCF group because radiographic leaks have been demonstrated to have low positive predictive value 14 -a number of patients deemed to have leaks on radiographic studies have had false positive results. Leak studies are often ordered based on clinician preference prior to oral feeding.…”

“…It is standard practice to conduct a fluoroscopic contrast swallow to determine whether a postoperative leak is present prior to commencing oral intake. A recent retrospective review of contrast swallows to determine leakage, conducted 1–3 weeks post operatively, demonstrated 50% sensitivity and 92.1% specificity, with a positive predictive value of 62.5% and a negative predictive value of 87.5% [16]. A randomized controlled trial is needed to determine best practice regarding timing and protocols to commence oral intake.…”

“…The reported incidence of perioperative mortality for patients with head and neck cancer undergoing TORS ranges from 0.07-3.3% (Table 1) (2,7,14,(16)(17)(18)(19). Although these studies provide a general estimate, no study has specifically examined perioperative mortality as a primary outcome of TORS using national or hospital-based data.…”
