The utility of ESR, CRP and platelets in the diagnosis of GCA

Abstract: Background To compare the utility of ESR, CRP and platelets for the diagnosis of GCA. Method A clinical diagnosis of GCA was determined by case-note review of 270 individuals (68% female, mean age 72 years) referred to a central pathology service for a temporal artery biopsy between 2011 and 2014. The highest levels of ESR, CRP and platelets (within 2 weeks of diagnosis) were documented. Evaluation of ESR, CRP and platelets for the diagnosis of GCA were compared using R… Show more

“…Thus, while elevated inflammatory markers are important, their absence does not rule out the disease. The sensitivities of ESR 9 50 mm/h, CRP 9 20 mg/L, and platelets 9 300 × 10 9 /L were 65.5%, 66.9%, and 71.2% respectively, while specificities were 57.3%, 67.9%, and 71.2% respectively in one study of 139 patients, when compared against physician-determined diagnosis [41]. In the same analysis, if just ESR and CRP were positive, with normal platelets, the odds ratio of GCA was 4.0, which increased to 10.7 if platelets were high as well.…”

“…Elevated CRP levels can be revealed before the clinical manifestation of inflammatory arthropathy [ 50 ]. The evaluation of the activity of systemic vasculitis are also based on the measurement of CRP levels [ 51 , 52 ]. An increased CRP level may be a predictive marker of systemic scleroderma progression for both skin lesions and pulmonary fibrosis [ 53 , 54 ].…”

“…Platelets are also considered a serum marker for inflammation in GCA. The postulated mechanism of thrombocytosis in promoting inflammation stems from their early interaction with the endothelium in inflammatory states during which they provide adhesion molecules and chemotactic stimulation to aid in the recruitment of leukocytes and enhance the release of different inflammatory mediators [10].…”
