The value of delayed phase enhanced imaging in malignant pleural mesothelioma

Abstract: Optimal MPM enhancement on MRI likely occurs at a time delay between 2.5-5 min following IV contrast administration. Further study of delayed phase enhancement of MPM with dynamic contrast enhanced MRI is warranted.

“…However, the correlation we observed between tissue MVD and MSIG in MPM (r = 0.4258, p = 0.02, n = 28) appeared preserved, and indeed more powerful, when this analysis was confined to patients without macro-nodules (r = 0.6594, p = 0.003, n = 18). Furthermore, the 4.5 min SI peak observed here is concordant with a recent retrospective MRI study involving MPM patients, in which peak tumour enhancement occurred at 280 s post-gadolinium administration [ 32 ].…”

“…A recent study analysed contrast enhancement dynamics of MPM in MRI [15]. The results of the study demonstrate that optimal contrast enhancement of MPM in MRI is likely to occur 2.5-5 minutes following intravenous contrast agent administration.…”

“…However, correct phases were ensured by checking the images for established characteristics of arterial and portal venous phase. Evaluation of further scan delays apart from arterial and portal venous phase would be desirable especially in light of the abovementioned results of MR-imaging showing the best delay being > 2.5 minutes [15]. However, when studying the results of the MR trial in detail, there was not much of increasing intensity beyond 70 seconds scan delay.…”

“…Recent research has shown that the normally used time delay between contrast medium administration and start of image acquisition might not be sufficient to capture contrast phases optimal for MPM assessment. Patel et al 22,23 showed that enhancement of MPM at the conventional time delay of 40 to 60 seconds did not represent the maximal enhancement. In fact, the group estimated that the peak tumor enhancement occurs at 280 seconds following intravenous (IV) contrast injection (►Fig.…”
