The Value of Radiographs and Bone Scintigraphy in Suspected Scaphoid Fracture

Abstract: The role of radiography and bone scintigraphy in the diagnostic management of suspected scaphoid fracture is controversial. Two strategies were compared for patients with initial negative radiographs: repeated radiography versus selective bone scintigraphy. Using the known positive predictive value of scintigraphy, the sensitivity and specificity of both diagnostic strategies were evaluated in a series of 78 consecutive patients. The kappa value for initial radiographs was 0.76 but decreased to 0.5 for follow-… Show more

“…42 Five studies also reported interrater kappa values for x-ray between radiologists and hand surgeons. [55][56][57][58][59] Several studies reported on x-ray imaging techniques that are not readily available in the ED, including carpal box x-rays, 60,61 digital contrast enhancement, 62 and dual x-ray absorption. 63 Waeckerle 42 reported that the initial x-ray sensitivity was 80% with 100% specificity (LR+ = infinity, LR-= 0.20).…”

“…Dias 57 noted between-resident interrater kappa of 0.30 compared to 0.43 for senior residents, 0.53 for orthopedic surgeons, and 0.80 for radiologists. Tiel-van Buul et al 55,56 reported interrater kappa 0.76 for the initial x-ray, but only 0.40 when the initial x-rays are negative. Low and Raby 59 reported kappa estimates ranging from 0.18 to 0.53 between four clinicians.…”

“…Scaphoid fractures can be difficult to diagnose on plain films, with reported sensitivities between 70% and 86%. 31 Hence in an acute setting, any suspected scaphoid injury should be managed with a splint. CT or MRI may be needed to make the diagnosis of subtle or stress fractures.…”

“…Patients who visited the Emergency Department with an acute trauma and a suspected scaphoid fracture after physical examination but no evidence of a scaphoid fracture on plain radiographs underwent at our institution bone scintigraphy [5][6][7][8][9][10][11]. We noticed that bone scintigraphy detected many other occult carpal and distal radius fractures in these patients with a suspected scaphoid fracture.…”
