The value of typing basal cell carcinomas in predicting recurrence after surgical excision

Abstract: A classification, based on growth pattern was devised and applied to 156 basal cell carcinomas from 134 patients. The tumours were divided into four main groups; (I) nodular; (2) nodular with infiltrative margin; (3) infiltrative; (4) multifocal. The infiltrative and multifocal types exhibited a high rate of recurrence following surgical excision whereas recurrence of nodular tumours was much rarer. It is therefore suggested that growth pattern should always be stated in the routine histopathological reporting… Show more

“…Overall, there were no significant differences between patients with CLL and controls in preoperative tumor size (median, 1.6 cm vs 1.4 cm; P=. 18) and proportion of aggressive histologic subtypes of BCC (58% vs 41%; P=.12).…”

“…[3][4][5] Aggressive tumors tend to be larger or to exhibit infiltrative features, findings that, in turn, predict higher recurrence rates. 18,19 Because numerous case series anecdotally suggest aggressive histologic behavior of nonmelanoma skin cancer in patients with CLL, larger BCCs with morpheaform, micronodular, and metatypical subtypes would be suspected to predominate in such patients. 12,[20][21][22] However, in our study, there was no significant difference between the case patients and the control patients in the median preoperative tumor size (1.6 cm vs 1.4 cm, respectively; P=.…”

“…2 There can also be mixed tumors, including a combination of any two or all of the above types. 3,4 Histological assessment remains the basis of a tumor analysis and this is especially true for BCC, where our understanding of its cellular defects remain limited. A key goal for cancer pathologists is to relate the cancer phenotype to a list of defining molecular principles.…”

“…Thus, BCC is considered to be a neoplasm composed of usually undifferentiated yet quite pluripotential germinative cells. Based on the overall growth pattern, the tumours may be classified in three main groups: superficial multicentric, nodular expansive, and infiltrative (Sloane, 1977 (Sloane, 1977;Jacobs et al, 1982;Leffel et al, 1991). We have previously reported that E-CD expression was preserved in cases of superficial and nodular BCC, and was reduced in most cases of infiltrative BCC, suggesting a role for E-CD in the growth pattern and local invasiveness of BCC (Pizarro et al, 1994).…”
