The Vecchietti operation for constructing a neovagina: Indications, instrumentation, and techniques

Abstract: The Vecchietti method is a surgical technique for the treatment of vaginal agenesis that constructs a dilation-type neovagina in 7-9 days. We have performed the Vecchietti operation on six patients. The method was applied to create a primary neovagina, to lengthen a surgically foreshortened vagina, and to reconstruct an obliterated neovagina with or without the presence of a uterus. Two patients underwent the conventional transabdominal approach, and four, laparoscopy. Follow-up was 7-39 months, and neovaginal… Show more

“…In the future a validated patient questionnaire will be offered to such patients. All except 1 patient resumed sexual activity with a median delay of 10 weeks (range, [8][9][10][11][12][13][14][15]. Vaginal sensation was reported as ''minimal,'' but penetration was described as satisfactory and painless in 6 of 7 patients, along with normal vulvar sensitivity.…”

“…We investigated surgical methods for corrections of vaginal aplasia (Mayer-Rokitansky-Kuster syndrome). Although Vecchietti's mechanical method [7] was obviously not adapted to our patients' situation, McIndoe's technique, consisting of the placement, between the rectum and vagina, of a mold covered by a splitthickness skin graft, which becomes the future neovagina, was considered [8]. The disadvantages of this procedure are the need to sample skin, creating a new visible scar(s), and the necessity of a long and strict bedrest to avoid graft mobilization and necrosis.…”

“…In the Vecchietti technique, traction rather than dilatation is needed to create a new vagina. Classical Vecchietti operation was an abdominal surgery that was done abdominally through a Pfannenstiel incision (14); however, this procedure was modified laparoscopically since it has high complication rates (15). This traction process may be too painful, and patients may not tolerate it easily (14,16,17 (22,23).…”

“…Numerous procedures were described for creation of neo-vagina with acceptable function, feeling and appearance. [5] They included serial dilation [6][7], Vecchietti's technique [8][9][10], sigmoid or ileal flaps [11][12][13][14][15][16], gracilis flap [17], Singapore flap [18][19] and expanded vulval flap [20]. Among these options the modified Mc indoe technique gained popularity being the simplest operation with very low donor site morbidity.…”
