The vertical posteromedial thigh (vPMT) flap for autologous breast reconstruction: A novel flap design

Abstract: The vertical vPMT is suitable for breast reconstruction in women with small to moderate breast size; the vertical pattern of the PMT avoids some of the problems related to other flaps with transverse patterns and represents an alternative design that can be successfully used for breast reconstruction. © 2016 Wiley Periodicals, Inc. “…The vena comitans of the PAP was anastomosed to the internal jugular vein in an end‐to‐side manner and the accessory saphenous vein was anastomosed to the external jugular vein in an end‐to‐end manner under the sternocleidomastoid muscle (Karakawa, Harima, & Iida, ) (Figure e). Two venous anastomoses were expected to contribute to the reduction in venous thrombosis and flap failure rates (Ahmadzadeh, Bergeron, Tang, Geddes, & Morris, ; Scaglioni, Chen, Lindenblatt, & Giovanoli, ; Scaglioni, Eder, & Giovanoli, ). The postoperative course was uneventful and the transferred flap completely survived (Figure f).…”

“…The residual concealed scar and low functional morbidity after harvest are well appreciated (Allen et al, ; Satake et al, ). Sufficient flap volume and high versatility have expanded their surgical indications (Fernandez‐Riera, Hung, Wu, & Tsao, ; Mayo, Canizares, Torabi, Allen, & St. Hilaire, ; Scaglioni, Chen, Lindenblatt, & Giovanoli, ; Scaglioni, Hsieh, Giovanoli, & Chen, ; Wu, Huang, & Cheng, ).…”

“…In their study, every patient underwent immediate breast reconstruction and the average weight of the vPMT flap was 431 g (range: 390-470 g). 19 The same team reported a new design of the vPMT flap as an inverted L shape to increase its volume to fit medium-sized breast in immediate breast reconstruction. In this single case report, the flap weight was 310 g. 20 In the current report, the mean fleur-de-lys PAP flap weight was 480 g versus 222 g for the horizontal PAP flap harvest.…”
