The vibratory pattern of ultrasonic files driven piezoelectrically

Ultrasonic irrigation of the root canal can be performed with or without simultaneous ultrasonic instrumentation. When canal shaping is not undertaken the term passive ultrasonic irrigation (PUI) can be used to describe the technique. In this paper the relevant literature on PUI is reviewed from a MEDLINE database search. Passive ultrasonic irrigation can be performed with a small file or smooth wire (size 10-20) oscillating freely in the root canal to induce powerful acoustic microstreaming. PUI can be an important supplement for cleaning the root canal system and, compared with traditional syringe irrigation, it removes more organic tissue, planktonic bacteria and dentine debris from the root canal. PUI is more efficient in cleaning canals than ultrasonic irrigation with simultaneous ultrasonic instrumentation. PUI can be effective in curved canals and a smooth wire can be as effective as a cutting K-file. The taper and the diameter of the root canal were found to be important parameters in determining the efficacies of dentine debris removal. Irrigation with sodium hypochlorite is more effective than with water and ultrasonic irrigation is more effective than sonic irrigation in the removal of dentine debris from the root canal. The role of cavitation during PUI remains inconclusive. No detailed information is available on the influence of the irrigation time, the volume of the irrigant, the penetration depth of the instrument and the shape and material properties of the instrument. The influence of irrigation frequency and intensity on the streaming pattern as well as the complicated interaction of acoustic streaming with the adherent biofilm needs to be clarified to reveal the underlying physical mechanisms of PUI.

An in vitro comparison of penetration depth of two root canal sealers: An SEM study

Aim:The aim of this study was to examine in vitro penetration depth of two resin-based sealers (AH plus and Resino Seal) and Zinc Oxide Eugenol sealer into the dentinal tubules after removing smear layer by passive ultrasonic irrigation.Materials and Methods:Thirty freshly extracted maxillary central incisors were used. The teeth were decoronated, working length established and prepared upto size 40 file. Each root was subjected to passive ultrasonic irrigation with 2.5% sodium hypochlorite. Three different sealers and gutta-percha were used for obturation. Roots were sectioned using hard tissue microtome. These sections were gold sputtered and examined under scanning electron microscope.Results:Statistical analyses of the data were performed using Kruskal–Wallis and Mann–Whitney tests. Statistically significant difference was found between AH Plus sealer and Resino Seal sealer and Zinc Oxide Eugenol sealer.Conclusion:The results showed that AH Plus had maximum penetration depth into dentinal tubules.

Introduction: The introduction of piezoelectric instruments in endodontic surgery allowed clinicians to manage the bone tissue with precision reducing the risk of damaging soft tissues. Although such instruments can also be used to prepare root-end cavity, few information is available on the effect of piezosurgery on dentine. This cadaveric study investigated the integrity of root apices after root-end cavity preparation by piezoelectric instruments at different device settings. Methods: Fifty maxillary anterior teeth underwent endodontic treatment and apical resection in situ. Ten teeth were used as control. Retrograde cavities were prepared in 40 teeth (10 per group). In three groups, the piezoelectric device was set at constant vibration mode (CV), and three power levels were used. In another experimental group (n = 10), vibration + pulsation mode (VP) was selected, and low power was used. Each root was duplicated and analyzed by scanning electron microscopy for the presence of cracks and marginal chipping. Results: The number of cracks was significantly higher in the VP group, whereas no significant difference was found among CV groups. No difference between groups was found for crack type. The VP group showed a significantly poorer quality of cavity margin respect to the CV groups. Conclusion: Constant vibration mode is recommended for retrograde preparation with piezosurgery.
