The Virtual Inclusive Digital Health Intervention Design to Promote Health Equity (iDesign) Framework for Atrial Fibrillation: Co-design and Development Study

Abstract: Background Smartphone ownership and mobile app use are steadily increasing in individuals of diverse racial and ethnic backgrounds living in the United States. Growing adoption of technology creates a perfect opportunity for digital health interventions to increase access to health care. To successfully implement digital health interventions and engage users, intervention development should be guided by user input, which is best achieved by the process of co-design. Digital health interventions co-… Show more

“…We used the iDesign framework, a virtually inclusive digital health intervention design approach to promoting health equity, to guide the HCD sessions in this study 21 . The iDesign framework includes 7 steps with varying sequence depending on the type or maturity of the project: (1) defining challenges and empathizing; (2) ideation; (3) virtual onboarding; (4) virtual prototyping; (5) ranking prototypes based on feasibility, viability, and desirability; (6) engineering work; and (7) testing (Figure).…”

The virtual onboarding phase was not conducted for this study given that the intervention is in the early design phase.…”

“…The Digital Optimization Toolkit to Improve Heart Failure Care toolkit was first developed for the United States and consists of a patient engagement application integrated with a clinician‐facing dashboard. Core elements of the toolkit were developed using HCD 44 and include assessment of patients' health status, medication adherence, and vitals through remote monitoring tools (Bluetooth‐enabled scale and blood pressure cuff). When communicated efficiently to clinicians, these data may promote timely and safe GDMT optimization and patient empowerment, which has increased GDMT optimization by nearly 20% in prior studies.…”
