SummaryLichen planus is characterized by lichenoid, polygonal papules with fine white lines, called Wickham striae. Lesions most commonly occur on the limbs and on the dorsal aspect of the trunk. At the same time often leukoplakia of mucous membranes as well as nail disorders are seen.There are numerous variants of lichen planus which can be distinguished from the classical form on the basis of morphology and distribution of the lesions. The typical primary lesion of lichen planus may be replaced by other forms, such as patches, hyperkeratoses, ulcerations, or bullous lesions. Moreover, distribution patterns of these lesions may vary and include erythrodermic, inverse or linear arrangements. In contrast to these numerous clinical features, histologic findings remain characteristic in the variants, so that the diagnosis can be made securely. Differential diagnoses of lichen planus include diverse dermatoses such as bullous pemphigoid or paronychia.

Abstract:Vulvovaginal-gingival syndrome (VVGS) is a chronic mucocutaneous disease of unknown etiology, which consists of a triad of symptoms: vulvar, vaginal and gingival of lichen planus (LP) lesions. The evidence suggests an autoimmune response, a genetic and probably hormonal component. Associated factors observed in LP include stress/anxiety, hepatitis C virus (HCV) and autoimmune diseases, among others. The most serious complication of oral and possibly also vulvar Lichen Planus is the development of squamous cell carcinoma. Corticosteroids are the mainstay treatment for LP in order to control inflammatory activity. In case of failure, immunosuppressive agents can be used (Tacrolimus and Cyclosporine). We present an illustrated case of VVGS who over the years developed tongue cancer and, 10 years later, a vulvar cancer. Oral Lichen Planus (OLP) should be referred to the appropriate specialists to rule out involvement of other mucous membranes and track it early to diagnose a possible malignant transformation. The efficient management of VVGS involves a multidisciplinary approach involving gynecologists, dermatologists and stomatologists.

Przegląd Dermatologiczny 2014/4 299 StreSzczenie Wprowadzenie. Zespół sromowo-pochwowo-dziąsłowy jest rzadką odmianą liszaja płaskiego, w której zajęte są błony śluzowe jamy ustnej, sromu oraz pochwy. Choroba najczęściej występuje u kobiet w wieku okołomenopauzalnym. W obrazie klinicznym dominują bolesne zmiany nadżerkowe. W leczeniu pierwszą opcję stanowią glikokortykosteroidy miejscowe i doustne. Nieleczona choroba może prowadzić do trwałych powikłań w postaci zrostów i zwężeń zajętych narządów. Dlatego bardzo ważna jest wczesna diagnoza i szybkie rozpoczęcie właściwego leczenia. Cel pracy. Przedstawienie przypadku pacjentki z zespołem sromowo--pochwowo-dziąsłowym leczonej metyloprednizolonem, cyklosporyną i miejscowo maścią z takrolimusem. Opis przypadku. Chora w wieku 54 lat, z kilkuletnim wywiadem dolegliwości ze strony jamy ustnej oraz narządów płciowych, dotychczas leczona ambulatoryjnie przez ginekologów i stomatologów z rozpoznaniem parodontozy i kandydozy. Pacjentka zgłosiła się do Kliniki Dermatologii w Lublinie, gdzie na podstawie obrazu klinicznego i badań dodatkowych ustalono rozpoznanie zespołu sromowo-pochwowo--dziąsłowego. W terapii zastosowano dożylny puls sterydowy, cyklosporynę A i miejscowo maść z 0,1% takrolimusu. Uzyskano wygojenie zmian nadżerkowych i ustąpienie dolegliwości subiektywnych. Wnioski. W diagnostyce i leczeniu zespołu sromowo-pochwowo-dzią-słowego konieczne jest podejście interdyscyplinarne. Należy brać pod uwagę tę chorobę w diagnostyce różnicowej przewlekłych stanów zapalnych błon śluzowych jamy ustnej i okolicy genitalnej z dominujący-mi w obrazie klinicznym nadżerkami. AbStrActIntroduction. Vulvo-vaginal-gingival syndrome is a rare variant of lichen planus. The disease affects mucous membranes of the oral cavity and genitals. It usually affects middle-aged females and most frequently presents as painful erosions. First-line treatment includes topical and systemic corticosteroids. If untreated, the disease may result in mucocutaneous scarring and stricture formation in affected organs. Therefore early diagnosis and appropriate treatment is essential. Objective. To present a patient with vulvo-vaginal-gingival syndrome treated with methylprednisolone, cyclosporin and tacrolimus ointment.
