The article notes features of the last epidemic season (2020-2021) in the form of extremely low activity of influenza viruses with SARS-CoV-2 remaining dominant in the etiological structure of acute respiratory viral infections. Presented own data indicating the heterogeneity of SARS-CoV-2 (Alpha, Delta, B.1.1.317, B.1.1.397, B.1.1.523) isolated from hospitalized patients was noted. An increase in the etiological role of bocavirus, alpha-coronavirus and metapneumovirus with a decrease in the frequency of parainfluenza viruses, adenoviruses, rhinoviruses and respiratory syncytial virus was established. An unusual shift of the period of maximum activity of respiratory syncytial virus to the 20th week was noted. Attention is paid to the need during the ongoing pandemic COVID-19 to fully immunize children as part of the National Immunization Calendar, also emphasized the role of mandatory annual vaccination against influenza. It is noted that the maximum protective effect of immunoprophylaxis of influenza acute and recurrent respiratory infections can be achieved with a combination of vaccination with bacterial lysates. The article presents a review of the literature and our data demonstrating the safety, good tolerability, and high clinical and immunological efficiency of polyvalent mechanical bacterial lysate for the prevention of acute and recurrent respiratory infections in children. It is shown that the use of polyvalent mechanical bacterial lysate during the prevaccination period makes it possible to substantially reduce the frequency of intercurrent infections, which reduces the number of temporary medical withdrawals and increases the coverage of vaccinations against influenza in organized groups to 85.1%. The role of trained immunity as one of the possible mechanisms providing nonspecific immunoprophylaxis during influenza vaccination and the use of bacterial lysates is discussed.

Introduction. All acute respiratory infections are characterized by damage to the mucous membrane of the respiratory tract, a decrease in local immune defense and a weakening of nonspecific protective factors.Aim. To characterize the clinical effectiveness of the use of the immunocorrective drug in frequently ill children.Materials and methods. The article assessed the microbiological spectrum of the rhinopharyngeal landscape in 65 children: 33 children took an immunocorrective drug in addition to conventional therapy – the main group; 32 children received conventional therapy (the comparison group), and also investigated the possibility of regulating their polypathogenic microbiota using topical bacterial lysates.Results and discussion. The result of the therapy was a clinical improvement characterized by the relief of pharyngoscopic signs of chronic tonsillitis in children while taking topical bacterial lysates in form of lozenges. Analysis of the microbiota of the oropharyngeal mucosa in the studied patients of both groups before the start of therapy revealed the species diversity of opportunistic flora, among which Streptococcus pyogenes, Staphylococcus aureus, the family Enterobacteriaceae and Candida albicans were identified in all patients in diagnostic significant titers of 105–1010. After treatment in the main group, the ratio of the microbiota of the nasopharynx significantly changed, stimulation of the production of local immunity components was noted, which was confirmed by a decrease in the intensity of contamination of microbiota with polypathogenic functions to diagnostically insignificant titers. When comparing the species composition of the microbiota in the studied groups after treatment with topical bacterial lysates, a statistically significant positive effect was found without additional use of antibacterial drugs.Conclusions. It has been shown that the immunocorrective drug eliminates pharyngoscopic signs of rhinopharyngeal dysbiosis in children who are often and long-term ill.
In childhood, the occurrence and steady growth of diseases of the nasopharyngeal tonsils, often acquiring a chronic course and occurring with exacerbations and complications, is facilitated by a number of factors. Etiological factors of adenotonsillar diseases in children are both bacterial and viral pathogens, among which respiratory viruses are among the most significant. Herpes virus infections (HVI) play an important role in the development of chronic upper respiratory tract pathology, especially in the cohort of frequently ill children. Representatives of the herpes virus family are characterized by lymphotropic action, long-term persistence in the body, and a tendency to reproduce in cells, leading to a violation of the immune status. The incidence of relapses in the postoperative period during surgical interventions in the ENT organs in children remains high. Herpesvirus etiology of recurrent infectious and inflammatory diseases of the URT (upper respiratory tract) is the leading factor in the ineffectiveness of tonsillotomy, adenotomy, and repeated proliferation of lymphoid tissue in the postoperative period in preschool children. The use of a drug from the group of bacterial lysates before surgery is justified in the treatment of frequently and long-term ill children with adenotonsillar hypertrophy.
