Therapeutic Management of Rare Malignant Pancreatic Tumors in Children

Abstract: Malignant pancreatic tumors in children are rare. The major problem for the clinician is a lack of experience and of accepted therapeutic strategies. Malignant pancreatic tumors in children show a pattern different from that in adults. In infants, especially pancreatoblastomas, solid cystic tumors of females, and endocrine carcinomas of the pancreas must be expected. We report our experience in three patients with malignant pancreatic tumors (one pancreatoblastoma and two malignant endocrine pancreatic carcino… Show more

“…As there are no reports of long-term survival in any patient who did not undergo resection, complete tumour removal is recommended [30,33,80,89] and is strongly supported by the results of the survival analyses undertaken in the present study. Complete resection was achieved in approximately two-thirds of the patients as reported in literature [77] and in 80% in the present study.…”

“…Direct extension may occur, including to the omentum, peritoneum, spleen, left kidney and left adrenal [10] and there may be vascular invasion [21,47,63]. It is has been said that the tumours are generally of low metastatic potential with a firm capsule even when the tumour is large [63,89]. Bone metastases [37,44] are very rare and our search found only 2 cases.…”

“…Pediatric patients with localized tumors can be cured by complete surgical resection. Children with metastatic disease have generally faired poorly, although recent reports of favorable responses to chemotherapy and radiation [163][164][165][166][167] suggest that even some of these patients may experience long-term survival. In adults, pancreatoblastomas have been nearly universally fatal, with most cases progressing quickly, similar to acinar cell carcinomas.…”

“…(1, 2 ,3, 4, 5) The main pancreatic tumors in children are pancreatoblastoma, papillary cystic tumors and malignant endocrine tumors of the pancreas. 6 These are usually managed surgically and evidence for the role of adjuvant therapy is nearly entirely anecdotal. Pancreatic trauma may need surgery for duct disruption or pseudocysts.…”
