Therapeutic role of endoscopic ultrasound in pancreaticobiliary disease: A comprehensive review

Abstract: With the development of technology and accessories, the role of endoscopic ultrasound (EUS) has evolved from diagnostics to therapeutics. In order to characterise the therapeutic role of EUS, we searched Web of Knowledge database and reviewed articles associated with therapeutic EUS. There are two modalities for the therapeutic purpose: drainage and fine-needle injection. EUS-guided drainage is a promising procedure for the treatment of peripancreatic fluid collection and biliary obstruction; EUS-guided fine-n… Show more

“…Recently, interventional EUS has come to occupy an important role in pancreaticobiliary drainage where conventional ERCP procedures fail. 19,20 During EUS-guided intervention for various therapeutic methods (e.g. management of pancreatic fluid collections, biliary/pancreatic duct drainage, and drainage of the gallbladder), unnecessary dilation of a fistula can increase the risk of fluid leakage.…”

“…It can also be used as an alternative to percutaneous drainage since it provides less trauma and a better quality of life in terminally ill patients [55]. EUS-guided biliary drainage is a novel technique that is still on a research stage.…”

“…Endoscopic ultrasound has also been used to target pancreatic cancer with other local therapies such as photodynamic therapy, radio frequency ablation, cryothermal ablation and high-intensity focused ultrasound probes [55,77,78]. All of these therapies are at early investigational stages and they are not ready for prime time just yet.…”

“…The fact that some organs could have only been accessed by surgical procedures has promoted endoscopy to a level worthy of further appraisal. Among the different steps in endoscopy, the ones that surely changed the way we tend to diagnose or treat patients in daily practice are endoscopic retrograde cholangiopancreatography [6], capsule endoscopy [7], and endoscopic ultrasound [8,9]. Thus, a new window was opened for both patients and physicians, and allowed the concept of evidencebased medicine to be used in daily practice.…”
