Background: Total meniscectomy for treating massive meniscal tears may lead to joint instability, cartilage degeneration, and even progressive osteoarthritis. The meniscal substitution strategies for advancing reconstruction of the meniscus deserve further investigation. Hypothesis: A decellularized meniscal scaffold (DMS) modified with collagen affinity stromal cell–derived factor (C-SDF1α) may facilitate meniscal regeneration and protect cartilage from abrasion. Study Design: Controlled laboratory study. Methods: The authors first modified DMS with C-SDF1α to fabricate a new meniscal graft (DMS-CBD [collagen-binding domain]). Second, they performed in vitro studies to evaluate the release dynamics, biocompatibility, and differentiation inducibility (osteogenic, chondrogenic, and tenogenic differentiation) on human bone marrow mesenchymal stem cells. Using in vivo studies, they subjected rabbits that received medial meniscectomy to a transplantation procedure to implement their meniscal graft. At postoperative weeks 6 and 12, the meniscal regeneration outcomes and chondroprotective efficacy of the new meniscal graft were evaluated by macroscopic observation, histology, micromechanics, and immunohistochemistry tests. Results: In in vitro studies, the optimized DMS-CBD graft showed notable biocompatibility, releasing efficiency, and chondrogenic inducibility. In in vivo studies, the implanted DMS-CBD graft after total meniscectomy promoted the migration of cells and extracellular matrix deposition in transplantation and further facilitated meniscal regeneration and protected articular cartilage from degeneration. Conclusion: The new meniscal graft (DMS-CBD) accelerated extracellular matrix deposition and meniscal regeneration and protected articular cartilage from degeneration. Clinical Relevance: The results demonstrate that the DMS-CBD graft can serve as a potential meniscal substitution after meniscectomy.

Articular cartilage defects caused by trauma or osteoarthritis remain a significant challenge in clinical practice due to their poor self-healing capacity. Current clinical approaches are generally limited by their complex procedures or certain postoperation complications. Therefore, it is crucial to develop an effective strategy for one-step cartilage repair with long-term functional cartilage regeneration. This study reports the creation of an injectable hydrogel composed of cartilage decellularized extracellular matrix (CdECM) and hyaluronic acid methacrylate (HAMA). In the presence of photoinitiator ruthenium (Ru)/sodium persulfate (SPS), visible light (450 nm) not only triggered the polymerization of CdECM and HAMA to form stable hydrogel in 1 min but also facilitated the tyrosine cross-links between biotissue and CdECM without any further moiety grafting, which cannot be achieved by routinely used photoinitiators and provides the hybrid hydrogel with bioadhesive properties. This dual-network hybrid hydrogel demonstrated robust adhesion strength to cartilage and significantly enhanced mechanical performance in comparison to that of CdECM or HAMA alone. Further in vitro culture demonstrated the promotive effect of hybrid hydrogel on encapsulated porcine bone marrow mesenchymal stem cells toward chondrogenic differentiation after 21 days of incubation compared with HAMA. Through in vivo subcutaneous implantation in a mouse model for two and 4 weeks, we further illustrated that the hybrid hydrogel has improved biocompatibility in comparison to HAMA alone. Thus, the presented hydrogel is a promising biomaterial for cartilage regeneration through minimally invasive arthroscopic injection in clinical practice.

In the clinic, inactivation of osteosarcoma using microwave ablation would damage the periosteum, resulting in frequent postoperative complications. Therefore, the development of an artificial periosteum is crucial for postoperative healing. In this study, we prepared an artificial periosteum using silk fibroin (SF) loaded with stromal cell-derived factor-1α (SDF-1α) and calcitonin gene-related peptide (CGRP) to accelerate bone remodeling after the microwave ablation of osteosarcoma. The prepared artificial periosteum showed a sustained release of SDF-1α and CGRP after 14 days of immersion. In vitro culture of rat periosteal stem cells (rPDSCs) demonstrated that the artificial periosteum is favorable for cell recruitment, the activity of alkaline phosphatase, and bone-related gene expression. Furthermore, the artificial periosteum improved the tube formation and angiogenesis-related gene expression of human umbilical vein endothelial cells (HUVECs). In an animal study, the periosteum in the femur of a rabbit was inactivated through microwave ablation and then removed. The damaged periosteum was replaced with the as-prepared artificial periosteum and favored bone regeneration. In all, the designed dual-factor-loaded artificial periosteum is a promising strategy to replace the damaged periosteum in the therapy of osteosarcoma for a better bone-rebuilding process.
