Thiazide diuretics and risk of knee replacement surgery among patients with knee osteoarthritis: a general population-based cohort study

Abstract: Objective: Thiazide diuretic use is associated with higher bone mineral density (BMD) and possibly lower serum magnesium levels than loop diuretic use, and both high BMD and low serum magnesium have been linked to high prevalent knee osteoarthritis. This study aimed to compare the risk of a clinically relevant endpoint, knee replacement (KR) surgery, among initiators of thiazide and loop diuretics. Design: Among patients aged 50 years with a diagnosis of knee osteoarthritis in The Health Improvement Network (T… Show more

“…Adjusted variables in the Cox proportional hazard regression models were included in these models. Second, a subject's knee replacement was likely to be scheduled a few months before the operation was performed, we introduced a 3-month as well as 6-month lag-time to exclude knee replacements that occurred during this period 46 . Third, to minimize potential misclassification of the duration of knee OA we performed an analysis restricted to participants with incident knee OA that was diagnosed when participants were enrolled into THIN database.…”

“…First, we introduced a 3-month lag time to exclude knee or hip replacements that occurred during this period (i.e., within the 3 months after the index date). 21 Second, we performed a marginal structural model to account for nonadherence to initiation of melatonin versus hypnotic benzodiazepines. 22 Third, we used a sequential regression method to impute missing data and generated the effect estimates from these imputed datasets.…”

“…THIN has been used on numerous occasions in pharmacoepidemiologic studies to examine health outcomes and mortality risk in patients with OA or to assess drug safety during the COVID-19 pandemic (22)(23)(24)(25)(26)(27). Symptoms, examinations, and diagnoses in THIN are recorded using a hierarchical clinical coding system called Read codes (28).…”
