Thoracic Endometriosis Unmasked by Ovarian Hyperstimulation for in vitro Fertilization

Abstract: Thoracic endometriosis syndrome is a well-described, rare manifestation of endometriosis. We present a case of a 35-year old woman undergoing controlled ovarian stimulation prior to in vitro fertilization (IVF) who developed bilateral hemorrhagic pleural effusions. She was initially diagnosed with ovarian hyperstimulation syndrome, a complication of infertility therapy; however, she was later found to have occult thoracic endometriosis. We describe ovarian hyperstimulation syndrome and review the manifestation… Show more

“…Details of the degree of responsiveness to ovarian stimulation and the specific forms of endometriosis detected at surgery were not reported. Finally, Halvorson et al (2012) described a case of symptomatic thoracic endometriosis diagnosed immediately after IVF. The woman developed symptoms suggestive of ovarian hyperstimulation syndrome (OHSS) 3 days after the retrieval of 30 oocytes.…”

“…A chest computed tomography showed bilateral pleural effusion and a subsequent thoracentesis revealed hemorrhagic pleural fluid. This case had 30 oocytes retrieved and developed OHSS with hemorrhagic pleural effusion as described above [ 14 ]. The second case developed pneumothorax after in vitro fertilization and embryo transfer.…”

“…Several independent large case series failed to highlight major detrimental effects [[5], [6], [7], [8], [9], [10]]. On the other hand, these reassuring data contrast with some case reports of severe complications [[11], [12], [13], [14], [15]]. The main characteristics of these cases are summarized in Table 1.…”
