Intrathoracic kidney is a partial or complete displacement of the kidney above the hemidiaphragm into the mediastinal compartment of the thorax. It is usually seen as an incidental finding discovered on chest radiograph or abdominal ultrasound. However computed tomography consents the correct detection of intrathoracic masses and defines their shape, size, and extent. We here report a case of ectopic thoracic kidney in a 22-year-old man who had a long history of scrotal discomfort associated with right varicocele. Frequently, this ectopia does not affect renal function and the stretched ureter provides good drainage. In literature, a small number of cases shows that varicocele is a possible mode of presentation of kidney tumors, but this is the first case of varicocele secondary to intrathoracic kidney ectopia.

“…Ptister-Goedek & Brunir classified thoracic kidney into four groups (19) : thoracic renal ectopia with closed diaphragm, eventration of the diaphragm (relaxation of the diaphragm); diaphragmatic hernia (subdivided into congenital diaphragmatic defects and acquired hernia such as Bochdalek hernia, with a percentage lower than 0.25%); and traumatic rupture of the diaphragm with renal ectopia (17) . Thoracic kidney is normally asymptomatic and most of the cases present thoracic tumors on chest X-rays requested for any reason other than the suspect of this anomaly (3)(4)(5)(6)(7)(8)(9)(10)(11)(12)(13)(14) . Thoracic ectopic kidney is a rare anomaly, the rarest of all renal ectopia types (p = 0.005%). Herein, we describe a case of thoracic ectopic kidney in an 83-year-old black man who, upon seeking medical attention, presented a clinical profile consistent with pulmonary emphysema. A chest X-ray was ordered, and the results showed evidence of a mass, which was then diagnosed (through computed tomography) as renal ectopia. The majority of thoracic ectopic kidney cases present as an intrathoracic tumor seen on chest X-rays ordered for reasons other than suspicion of this anomaly and do not require special treatment.

As shown by some studies [5][6][7] that considered the use of preoperative MR in normal kidneys affected by renal cancer, MR can be a substitute for angiography in depicting the renal vessels before nephrectomy. The occurrence of renal cell carcinoma is an exceptional phenomenon in an ectopic kidney. We describe a patient with renal cell cancer of the pelvic kidney who underwent radical nephrectomy at our institute. Preoperatively the patient underwent paramagnetic medium-enhanced magnetic resonance that provided an accurate description of the ectopic renal vessels. During surgery the anatomy of the vessels appeared exactly as described by the MR examination. Histological examination revealed a pT2N0GIV renal cell cancer. Nine months later the patient underwent control examinations, and no delayed complications had occurred. In our experience magnetic resonance provided an exact description of the ectopic kidney vascularization as confirmed by the surgical findings. Preoperative knowledge is important both for the surgeon during the dissection of the vessels and to plan correct lymphadenectomy on the basis of the vessel source identified at imaging.
