Three component cartilage framework reconstruction for correction of post-traumatic nasal septal collapse

Abstract: Background:Post-traumatic nasal septal collapse results in flat, broad, non-projecting nose due to mid-vault collapse. These patients may have airway obstruction due to poorly supported internal valve. Traditional techniques like 'cantilever graft technique' or conventional 'L-graft technique' produce a rigid lobule as the single unit framework extends to the tip. These grafts also lack in internal valve support.Materials and Methods:Twelve patients with post-traumatic nasal septal collapse were treated with t… Show more

“…These studies comprised 1041 patients of whom 741 received autologous grafts and 293 received IHCC grafts. Studies were published between January 1, 1990, and December 31, 2017; the mean sample size was 36 patients (range, 9-157 patients); and studies were from countries around the world, including Turkey (n = 9), 3,[23][24][25][26][27][28][29][30] the United States (n = 5), 9,13,[31][32][33] South Korea (n = 4), 14,18,19,34 Italy (n = 2), 35,36 India (n = 2), 37,38 the United Kingdom (n = 1), 39 Germany (n = 1), 40 the Netherlands (n = 1), 15 Egypt (n = 1), 41 South Africa (n = 1), 42 and Saudi Arabia (n = 1). 43 Mean follow-up time ranged from 6 to 48 months in the studies that reported follow-up, although 8 studies did not.…”

“…Although conchal cartilage is criticized for its convolutions and its curved nature, this can be overcome by crosshatching, flattening, shaving of tubercle, and cutting it into multiple pieces followed by rearranging it to make a smooth surface (►Fig. 4).…”

“…Patients requiring structural reconstruction (excluding columellar strut), reconstruction and support for nasal ala, lateral wall, patients requiring spreader grafts, patients with past history of septal surgery or positive tip touch sign indicating loss of septal volume, and cleft lip nose patients were excluded. 4 A preoperative soap template to fill the deficiency was made. This was used as a guide tool for estimating the volume of the graft.…”

“…[25][26][27][28][29] To restore the deficiency of the septal bone, rhinosurgeons most often use autotissues -removed areas of the cartilage and bones of the septum of the nose. [30][31][32] In the treatment of curvature of the caudal septum, such methods as suturing, hinged door, septal rail, ethmoid sandwich graft, tongue in groove and extracorporeal septoplasty have been used. 33,34 The treatment of nasal septal deformity, despite the long history of development and many modifications of the classical operation, is one of the urgent problems of modern rhinosurgery.…”
