Three different surgical techniques of crown lengthening: A comparative study

Abstract: Introduction:A short clinical crown may lead to poor retention form thereby leading to improper tooth preparation. Surgical crown lengthening procedure is done to increase the clinical crown length without violating the biologic width. Several techniques have been proposed for clinical crown lengthening which includes gingivectomy, apically displaced flap with or without resective osseous surgery, and surgical extrusion using periotome. Objective:The aim of this paper is to compare clinically the three differen… Show more

“…Surgical crown lengthening helps to increase the clinical crown length without affecting the biologic width. 1 Crown-lengthening surgery involves various techniques, including gingivectomy or gingivoplasty or apically positioned flaps, which may or may not include osseous resection. Periodontal crown lengthening is also used in cases of delayed passive eruption, teeth with fractures, subgingival caries or both.…”

“…The crown-lengthening surgery is performed to increase the clinical crown length without violating the biologic width [ 2 ]. Several techniques have been proposed for clinical crown-lengthening, including gingivectomy, apically displaced flap with or without resective osseous surgery, and surgical extrusion using a periotome [ 2 ]. Forced tooth eruption via orthodontic extrusion is the technique of choice when clinical crown-lengthening is necessary in the esthetic zone [ 14 – 16 ].…”

“…Forced tooth eruption via orthodontic extrusion is the technique of choice when clinical crown-lengthening is necessary in the esthetic zone [ 14 – 16 ]. Some authors [ 2 , 14 , 15 ] affirm that, after clinical and radiographic evaluation, the surgical extrusion technique offers several advantages over the other conventional surgical techniques such as preservation of biologic width, interproximal papilla, and gingival margin position. Additionally, it maintains the esthetics, prevents marginal bone loss, and exposes sound tooth structure for the placement of restorative margins [ 14 , 15 ].…”

“…When the margin of a restoration approaches the alveolar bone, the biologic width is violated (Nethravathy, Vinoth & Thomas, 2013;Rosenberg, Cho & Garber, 1999). The biological width is the area of the gingival tissue connected to the tooth above the height of the alveolar bone (Garguilo, Wentz & Orban, 1961).…”
