Three Lectures on Schizophrenia

Abstract: It is well known that in some small European countries there exist especially favourable conditions for certain types of research which elsewhere meet enormous difficulties. This is exemplified in Denmark and other Scandinavian countries by the famous studies on the influence of genetic and environmental factors in schizophrenia, which were made possible by the exceptional availability of well organized national registers on twins, adoptees and psychotics.

“…These observations may be similar to the step-like changes found following each relapse. which have been described in a number of longitudinal studies (Bleuler, 1978(Bleuler, , [1972; Ciompi, 1980;Kraepelin, 1913;Snezhnevsky, 1966;Watt et al, 1983).…”

“…More than 80% of the sample showed some degree of social disability (global evaluation scores 1 to 4), and almost 30% were severely disabled in their global social functioning (score 4). Ciompi 11 ,in a long-term follow-up study of 289 schizophrenic patients in Switzerland, found that the overall social adaptation appeared to be good or fair in only one-third old the sample by the end of the study. In a study of 70 first episode schizophrenics in Germany, Schubart et al 37 reported that two-thirds had DAS global scores of 3 (poor adjustment) or more at 2-year follow-up.…”

“…Recovery in this context has been defined as the amelioration of symptoms and other deficits associated with the disorder to a sufficient degree that they no longer interfere with daily functioning, allowing the person to resume personal, social, and vocational activities within what is considered a normal range. What these studies suggest is that at least one quarter, and up to two-thirds, of people diagnosed with a serious mental illness will achieve this form of recovery from the disorder and its effects (e.g., Ciompi, 1980;Davidson, Harding, & Spaniol, 2005;Davidson & McGlashan, 1997;Harding, Zubin, & Strauss, 1987;McGlashan, 1988).…”
