Three-year longitudinal population-based volumetric MRI study in first-episode schizophrenia spectrum patients

Abstract: Brain volume abnormalities that have been consistently observed at the onset of non-affective psychosis may not inevitably progress, at least over the first years of the illness. Taking together with clinical and cognitive longitudinal data, our findings, showing a lack of brain deterioration in a substantial number of individuals, suggest a less pessimistic and more reassuring perception of the illness.

“…In these cases, as reported in Methods and Materials, we entered their results as if they were from separate studies. The same criterion was adopted for two studies (19,39) with multiple follow-up evaluations and reporting GM volume at each follow-up time point and for which antipsychotic intake between each follow-up period was derivable. Thus, a number of independent samples of patients higher than the number of original studies were analyzed.…”

“…Meta-regressions were performed when at least 10 independent studies were available for the outcome of interest. Thus, the potential moderating impact on GM changes of such variables as patient's IQ [reported only in three studies (20,36,37)] or DUP [reported only in three studies (26,38,39)] was not considered.…”

“…Another longitudinal study found that first-episode schizophrenia patients show widespread gray matter volume reductions over the first 1.5 years of illness (Asami et al, 2012). However, another 3-year longitudinal study of first-episode schizophrenia patients found no significant progressive volume decreases in schizophrenia compared with healthy controls, in all brain areas except the caudate (Roiz-Santianez et al, 2014). Thus, the length of the period for progressive brain changes to occur is unknown.…”

“…Whether these brain abnormalities might be static or progress over time is still under debate. For instance, some longitudinal studies have found nonprogressive brain volume change over the lifelong course of the illness [10,11,12], whereas other authors have shown evidence for progressive volume changes over time [13,14,15]. Variation in certain genes could partly explain these morphological discrepancies observed among studies with longitudinal design and follow-up of patients with psychosis.…”
