“…These efforts included work in a variety of surgical disciplines including breast [9, 13, 14, 16, 17, 32, 36], cardiovascular [11, 26, 27, 30, 33], gastrointestinal [10], head and neck [22, 35], hepatic [12, 20], neuro- [29], orthopedic [21, 37, 40, 41], pediatric [12], noncardiac thoracic [25, 30, 31, 33, 34], and vascular surgery [15, 18, 19, 23, 24, 38, 39]. This body of work emphasized the evolving versatility of fibrin sealant because it is capable of causing blood to clot, creating a sealing barrier, and gluing tissues together.…”

“…19 The lack of comprehension of this fact may be responsible for these ''contradictory'' results reported in the literature. This was further confirmed by Drake et al 20 and Frost-Arner et al, 21 who demonstrated that the application of lower concentrations (500 IU/ml) of thrombin in conjunction with fibrinogen, did not increase thrombogenicity in epigastric free flap models.…”

“…This experiment was not the only one to test the fibrin glue in free flaps' microvascular anastomoses, however, it was the more realistic one. In previous studies, 1,3,10,18,20,21 after the flap elevation, the pedicle was ligated and the flap was repositioned in its own bed. Consequently, all the anastomoses were performed between perfectly matching donor and recipient vessels, and always in an end-to-end fashion.…”

“…A thrombin-based preparation of fibrin sealant has recently been shown to have deleterious effects on microvascular anastomosis in an animal model [11] . Fibrin sealant constructed with a high concentration of bovine thrombin (1,000 IU/ml) was detrimental to microvascular patency when applied to the anastomosis in a rat free flap model [12] . For microvascular anastomosis, lower concentrations of thrombin or a sealant devoid of thrombin seem to be best for microvascular anastomotic patency.…”
