Thymoglobulin Induction Decreases Rejection in Solitary Pancreas Transplantation

Abstract: Thymoglobulin induction regimen led to a low incidence of acute rejection and a high rate of graft survival in solitary pancreas transplants. In addition, surveillance biopsies were useful in the detection of early acute rejection in the absence of biochemical abnormalities.

“…While there have been considerable refinements in the surgical procedure, technical complication rates remain high after pancreas transplants. Improvements in immunosuppression have led to dramatic decreases in the immunologic graft loss rate (i.e., that due to acute or chronic rejection) (6). However, improvements in surgical techniques have resulted in only modest decreases in the TF rates.…”

“…With improved immunosuppressive regimens, acute rejection rates have decreased and graft survival rates have improved in these patients (6). To assess renal function in PTA patients, our protocol is to monitor serial measurements of serum creatinine, and also to determine glomerular filtration rate (GFR) levels using iothalamate clearance.…”

“…One-year graft survival rates after pancreas transplants have improved dramatically in the last decade, thanks to two main factors: improved surgical techniques, leading to a decreased incidence of graft loss to technical failures (1), and improved immunosuppressive regimens, leading to a decreased incidence of graft loss to acute rejection (2,3). Our armamentarium of drugs has grown significantly in this same decade.…”
