Timing of blood pressure medications and intradialytic hypotension

Abstract: Intradialytic hypotension (IDH) is a prevalent yet serious complication of hemodialysis, associated with decreased quality of life, inadequate dialysis, vascular access thrombosis, global hypoperfusion, and increased cardiovascular and all‐cause mortality. Current guidelines recommend antihypertensive medications be given at night and held the morning of dialysis for affected patients. Despite little evidence to support this recommendation, more than half of patients on dialysis may employ some form of this me… Show more

“…It’s crucial to be cautious with antihypertensive medications, as some might exacerbate the risk of IDH. As many as 50–90% of ESKD patients suffer from hypertension [ 167 ], and finding the balance between antihypertensive management and IDH risk can be a significant challenge due to fluid removal during dialysis [ 25 ]. Excessive fluid volume is a common cause of high blood pressure in dialysis patients, and the use of antihypertensive drugs needs careful management to avoid exacerbating IDH.…”

“…Itʹs crucial to be cautious with antihypertensive medications, as some might exacerbate the risk of IDH. As many as 50-90% of ESKD patients suffer from hypertension [164], and finding the balance between antihypertensive management and intradialytic hypotension (IDH) risk can be a significant challenge due to fluid removal during dialysis [13]. Excessive fluid volume is a common cause of high blood pressure in dialysis patients, and the use of antihypertensive drugs needs careful management to avoid exacerbating IDH.…”

“…Nighttime dosing of these medications is being suggested to minimize the risk. 58 At the same time, use of antihypertensive drugs with negative chronotropic effects and alpha blockers are described to exacerbate IDH by impairing the compensatory responses. 59 , 60 These effects are more significant in the setting of acidosis due to suppressed cardiac contractility.…”
