The present study aimed to evaluate the procoagulant effects of phosphatidylserine (PS) exposure on blood cells and microparticles (MPs), and examine its role in predicting early recurrence atrial fibrillation (ERAF) in patients with atrial fibrillation (AF) treated with pulmonary vein isolation (PVI). Blood samples were obtained from 40 healthy controls and 56 patients with AF at baseline (prior to PVI), and 0, 1 h, 1 day, 3 days and 7 days following PVI. The exposure of PS (PS+) to blood cells (platelets, erythrocytes and leukocytes) and MPs was detected using flow cytometry. The procoagulant activity was evaluated by coagulation time, and the formation of factor Xa (FXa) and thrombin. In addition, independent factors associated with PS+ blood cells and MPs, and significant predictors of ERAF following PVI were investigated by statistical analyses. The numbers of PS+ blood cells and MPs were significantly increased by PVI (P<0.01). A significant decrease in coagulation time, and increases in FXa and thrombin were exhibited in the PS+ blood cells and MPs from patients with AF treated with PVI, whereas these alterations were inhibited by either lactadherin or anti-tissue factor (P<0.01). The maximum power of the PVI was significantly associated with platelet-derived MPs, and high-sensitivity C-reactive protein (hs-CRP) was closely associated with leukocyte-derived MPs and endothelial-derived MPs (EMPs) (P<0.01). In addition, hs-CRP and EMPs >355/µl were identified as independent predictors of ERAF (P<0.05). The increased numbers of PS+ platelets, erythrocytes, leukocytes and MPs contributed to the procoagulant state of AF, and hs-CRP and EMPs were able to predict ERAF following PVI.

BACKGROUND: Senescence, a mechanism of cellular aging, which is characterized by irreversible proliferation arrest and a proinflammatory secretory phenotype, has been documented in women with preeclampsia. As cellular senescence can persist and progress, we postulated that it is associated with accelerated aging phenotype and accumulation of comorbidities in women with a history of preeclampsia. METHODS: We included a cohort of women with a history of preeclampsia (n=40) age- and parity-matched to a group of referent women with normotensive pregnancies (n=40). Women with prior major cardiovascular events, neurological, or autoimmune conditions were excluded. We collected urine and blood samples to study markers of aging, data on multimorbidity at the time of enrollment, and prospectively followed them for events over the course of 6 years, on average. RESULTS: Women with a history of preeclampsia exhibited unfavorable aging profiles compared with referent women, including decreased urinary α-Klotho ( P =0.018); increased leptin ( P =0.016) and leptin/adiponectin ratio ( P =0.027), and increased extracellular vesicles positive for tissue factor ( P =0.025). Women with a history of preeclampsia likewise had a higher rate of comorbidities at the time of enrollment ( P =0.003) and had a 4× higher risk of developing major cardiovascular events compared with referent women ( P =0.003). CONCLUSIONS: Our data suggest that a history of preeclampsia is associated with accelerated aging as indicated by senescence marker differences and the accumulation of multimorbidity later in life. Targeting cellular senescence may offer novel, mechanism-based approaches for the diagnosis and treatment of adverse health outcomes in women with a history of preeclampsia.

Pre-eclampsia (P-EC) is a major cause of maternal and neonatal mortality and morbidity. Despite intensive research, its aetiology remains poorly understood. However, underlying maternal cardiovascular risk factors are thought to be implicated. Changes in the maternal vasculature and coagulation profile may predispose women with P-EC to subsequent adverse cardiovascular consequences. Here we investigate the relationship between circulating levels of haemostatic factors and inflammatory cytokines in women with a previous history of P-EC. The participents included 26 women who had had P-EC within the last three years and were more than 6 months postpartum and 14 age-matched healthy women with no past history of P-EC. Blood was collected and assayed for plasma IL-6, IL-8, TNF-α and IL-10, Tissue Factor (TF) and TF-Pathway Inhibitor (TFPI), using Enzyme-Linked Immunosorbent Assays. Individually, plasma TF, IL-6, IL-8 and IL-10 levels increased in the P-EC group compared with their normal counterparts, whereas plasma TFPI and TNF-α level were reduced. Plasma TF/TFPI ratios and IL-10 values were significantly increased in the P-EC group compared with controls (p<0.05, p<0.01, respectively). There were positive and significant correlations between TFPI and IL-10 (r= 0.5; p<0.01) and TF/TFPI ratio and IL-10 (r= 0.31; p<0.041), and between IL-6 and TNF-α (r=0.71; p<0.001) and IL-6 and IL-10 (r=0.42; p<0.01). In conclusion, our results suggest the presence of elevated inflammatory cytokines and an imbalance of the haemostatic system in women with a past-history of P-EC, which may contribute to the known increased risk of cardiovascular disease in these women later in life.
