Tobramycin and Beta-Lactam Antibiotic Use in Cystic Fibrosis Exacerbations: A Pharmacist Approach

Abstract: OBJECTIVES: Survey suggests that recommended doses and dosage regimens for antipseudomonal antibiotics for the treatment of acute pulmonary exacerbations in cystic fibrosis (CF) patients are not used, and one way to address these disparities is the involvement of pharmacists who are dedicated to CF. This is the first survey specifically designed for pharmacists at Cystic Fibrosis Foundation (CFF)-accredited centers to identify how tobramycin and antipseudomonal beta-lactams are being used. The purpose of this … Show more

“…In clinical care, MIC testing is often not performed, or if it is, results are not back for several days after start of treatment. Therefore, AUC 24 alone is routinely utilized to guide dose individualization for tobramycin in patients with CF 3 . Traditional approaches to estimating AUC 24 utilize log‐linear regression (LLR) equations based on two timed tobramycin concentrations 13 .…”

“…A limitation of the study is the low response rate (45%) of pharmacists affiliated with CFF-accredited care centers; however, the response rate is comparable to that of previous antibiotic utilization surveys (42%-58%). 6,[15][16][17][18] Since over half of the pharmacists did not respond, the results may not be generalized.…”

“…Once-daily dosing for aminoglycosides is preferred [68]. The most commonly used beta-lactams in CF Foundation-accredited centers include ceftazidime, piperacillin-tazobactam, meropenem, and cefepime [74].…”

“…Some suspect this is due to lower protein binding and alterations in the volume of distribution due to chronic malnutrition [86]. Two commonly classes of antibiotics prescribed during a PEx, antipseudomonal beta-lactams and aminoglycosides, have interesting pharmacokinetic implications [74]. Although beta-lactam serum drug levels are not routinely measured, significant changes in antibiotic clearance can occur during PEx.…”
