Tomographic thallium-201 myocardial perfusion scintigrams after maximal coronary artery vasodilation with intravenous dipyridamole. Comparison of qualitative and quantitative approaches.

Abstract: SUMMARY Eighty-six patients had thallium-201 (20'Tl) myocardial perfusion scintigrams after intense coronary artery dilation with i.v. dipyridamole. The dipyridamole was infused at 0.15 mg/kg/min for 4 minutes. Significant side effects were infrequent. Coronary angiography showed that 24 of our 86 patients had normal coronary vessels or -30% luminal obstruction in one coronary artery; 11 patients had a moderate coronary obstruction (30-55% diameter narrowing or systolic compression) in one major vessel; and 51… Show more

“…This is illustrated in Figure 1. Of the 54 vessels with complete occlusion, 40 involved the infarct, and 14 involved the noninfarct zones (Table 5). Forty-three of these 54 occluded vessels (80%) were supplied by collaterals, which were present in 78% of infarct and 86% of noninfarct arteries.…”

“…Dipyridamole, which increases interstitial adenosine levels by the combined effects of inhibition of adenosine uptake and inhibition of its breakdown by adenosine deaminase, has been safely used for many years, especially in patients with poor exercise tolerance and capacity [14, 15, 16]. Dipyridamole does generally not cause significant elevation in the rate-pressure product, does not increase oxygen demand, and myocardial ischemia is rarely precipitated [17, 18].…”

“…The following end points are gen¬ erally applied to dobutamine stress echocardiography study and should lead to termination of testing: (1) de¬ tection of a new regional wall mo¬ tion abnormality or absence of endocardial thickening involving two or more wall segments12; (2) (20%). They found no statistical differences between the hypotensive and the nonhypotensive groups in prevalence or severity of CAD or in prognosis during 15 months of fol¬ low-up.…”
