Tools for monitoring patients with hepatocellular carcinoma on the waiting list and after liver transplantation

Abstract: For patients who are initially being listed for liver transplantation and for patients who are seeking to retain their active status on the wait list, the first step in the diagnostic consideration of hepatocellular carcinoma (HCC) is the accurate identification and staging of the tumors. Advances in imaging and biomarkers have improved our ability to diagnose HCC and in parallel have reduced the incidence of incidental HCC after liver transplantation. For the initial diagnosis, high sensitivity and high speci… Show more

“…Roberts [82] suggest that screening all patients for HCC recurrence after transplantation, using both imaging and serum biomarkers, is probably not cost effective. However, AFP monitoring, in itself, is not very costly and may be appropriate at regular intervals [83] . Yamashiki et al [78] proposed to measure AFP at monthly periods for the first two years after LT, to detect any HCC recurrence.…”

“…Liver imaging tests followed guidelines with the utilization of CT scans of at least 8-detector rows and 2-mm slices. Magnetic resonance imaging (MRI) was performed in scanners of at least 1.5T (21,22). Patients with HCC and with clinically significant portal hypertension, multinodular disease, impaired liver function, and those who demanded extensive hepatic resections were deemed unresectable and included on the waiting list.…”

“…Because the AFP level is increased in approximately 70% of HCC cases before LT, it is perhaps more useful than imaging for serially monitoring patients for recurrence after LT. This is particularly true if the AFP level normalizes and subsequently increases after LT. 2 The typical sites (ie, the lungs, liver, adrenal glands, lymph nodes, and bone) were not involved in this case, nor was the hepatic vein or inferior vena cava. Thus, this represented a true cardiac hematogenous metastasis, which is rare Figure. 1.…”
