Topical 5‐fluorouracil in dermatologic disease

Abstract: Topical 5-FU is used in a variety of dermatologic disease processes with a wide range of efficacy and levels of evidence. Based on extent and level of evidence, our disease-specific systematic review found that the evidence is strongest for topical 5-FU use in the treatment of actinic keratosis, squamous cell carcinoma, and basal cell carcinoma. This review serves as a comprehensive summary of topical 5-FU use in dermatology.

“…When entering the cell, 5-FU inhibits thymidylate synthase, thereby blocking DNA synthesis. 1 , 3 , 4 This disruption preferentially targets rapidly proliferating cells. 3 , 4 Patients typically report brisk dermatitis, which includes pain, pruritus, erythema, irritation, erosion, crusting, and eczematous reaction.…”

“…A number have achieved moderate success in the treatment of AK, such as topical 5-FU, imiquimod, ingenol mebutate, and diclofenac, which are all FDA approved for this indication (95). With regards to cSCC, topical 5-FU is also commonly used (96). Additionally, early randomized controlled trials show a high degree of clinical benefit from topical imiquimod in the context of cSCC, with approximately a 70% complete response rate (97,98).…”

“…Human skin provides the most comfortably accessible route of drug administration. Topical administration is the route of choice for cutaneous pathologies like atopic dermatitis since it rarely presents systemic adverse effects when compared to other routes of drug administration [ 1 ]. The stratum corneum of the skin, composed of flat dead cells encompassing high keratin filaments surrounded by a lipophilic matrix consisting of keratin, ceramides, cholesterol, cholesterol esters, and various other fatty acids, provides a natural physical barrier against particle penetration [ 2 , 3 ].…”
