Topical Antiglaucoma Medications and Lacrimal Drainage System Obstruction

Abstract: Patients taking a combination of topical antiglaucoma medications showed significantly increased risk of developing lacrimal drainage system obstruction. Both total and upper obstruction was significantly more frequent in patients on topical antiglaucoma medications.

“…Preoperative subclinical inflammation induced by previous exposure to long-term topical ocular glaucoma medication was felt to be the cause of this inflammation and higher surgical failure rate. Punctal stenosis is not uncommon from chronic exposure to topical ocular medication [40, 41]. In the NR, there are numerous cases of unilateral punctual stenosis occurring only on the side of the patient's monocular glaucoma.…”

“…In another investigation, Guzek et al 12 assessed strength and weakness of lacrimal tests including FDDT in patients with partial NLDO and claimed that an abnormal FDDT essentially confirms obstructive epiphora. 13 Kashkouli et al 8 studied on topical antiglaucoma medications associated with lacrimal drainage obstruction in adults with mean age of almost 60 years and reported that sensitivity and specificity of 5-minute FDDT were 92% and 82.7%, respectively. Similar to this study, Bowyer et al 13 also found that sensitivity and negative predictive value of FDDT in patients with congenital NLDO decreases but specificity and positive predictive value increases in time.…”

“…8 To the best of the authors' knowledge, there is no study evaluating FDDT sensitivity and specificity in adults with NLDO, which was the first aim of this study. The authors previously found a 92% sensitivity and 82.7% specificity for 5-minute FDDT in lacrimal drainage system obstruction associated with topical antiglaucoma medications.…”

“…Docetaxel has been reported to cause epiphora due to canalicular stenosis 66. The mechanisms of drug-induced lacrimal drainage obstruction may be related to the medications themselves, preservatives or duration of topical treatment 64. This condition has been proposed to be either dose-related or idiosyncratic 19.…”
