Topical isotretinoin in Darier's disease

Abstract: Topical 0.05% isotretinoin (Isotrex gel) was used to treat a test patch of skin in 11 patients with Darier's disease. Hyperkeratosis and papules improved in six patients after treatment for 3 months. Erythema, burning and irritation were common adverse effects, and these were severe in three patients, one of whom stopped treatment. Patients with mild Darier's disease may find topical isotretinoin helpful, but it is likely that most patients with widespread disease will require treatment with systemic retinoids. Show more

“…26 Topical therapies, such as steroids or vitamin D3 ointment, are widely used for the treatment of DD. 22 In addition, isotretinoin, 27 tazarotene 28 and adapalene (naphthoic acid) 29 have been evaluated as treatments for DD symptoms. For refractory proliferative lesions, surgical dissection using an yttrium-aluminum-garnet laser is sometimes performed.…”

“…Use of accutane/isotretinoin, a 13-cis retinoic acid formulation, commonly prescribed for the treatment of nodular acne, has proven to provide temporary symptomatic relief in DD [1,10,60]. Withdrawal from an accutane regimen has been reported to cause the return of DD symptoms [10,60]. Although, it is not known how accutane functions in DD, our results indicate that in DD keratinocytes, isotretinoin suppresses cell proliferation and perhaps promotes apoptotic cell death as a result of a reduction in TRPC1-mediated Ca 2+ influx [44].…”

“…Use of oral and topical retinoid therapy seems to be promising at present, albeit with a modest rate of success.
