Topical Tretinoin (Retinoic Acid) Therapy for Hyperpigmented Lesions Caused by Inflammation of the Skin in Black Patients

Abstract: Topical application of tretinoin significantly lightens post-inflammatory hyperpigmentation and, to a clinically minimal but statistically significant degree, lightens normal skin in black persons.

“…Prepuce Histology diminished intensity of hyperpigmentation seen in mucosal grafts. Hyperpigmentation may be solely the result of an inflammatory response, which was proposed previously for skin grafts, 21,22 although this assumption has been opposed by other authors. 23,24 It is clear that the exact mechanisms influencing this particular feature deserve further studies.…”

“…However, the hydroquinone group had a statistically superior reduction in hyperpigmentation at 6 months. These agents, in addition to rigorous sun protection, are thought to suppress melanocytes (Bulengo-Ransby et al, 1993) prior to the peel and hence prevent hyperpigmentation. Some recommend continuing this regimen post-peel as well for maximal efficacy, and a similar approach is used to treat hyperpigmentation resulting from a peel.…”

“…This further confirms what has been found in previous PIH and melasma studies, where histology was found to be ineffective, but spectroscopy was effective in quantifying pigmented skin from normal skin. 19,20 Thus, the correlation between clinical and spectroscopic evaluation suggests that these two components can be part of future clinical research to assess the efficacy of various interventions done to treat or prevent PIH.…”
