Objective This study was performed to assess the outcomes of Asian patients who underwent conversion from metal-on-metal total hip arthroplasty (MoM-THA) to cemented THA (CTHA). Methods One hundred and fifty-seven consecutive patients (157 hips) who underwent CTHA following primary MoM-THA from January 2005 to February 2015 were retrospectively analysed. The primary endpoints were the clinical outcomes. Follow-ups occurred at 3 months, 6 months, 1 year, 2 years, and then every 2 years following revision of MoM-THA. Results The mean follow-up after conversion was 10 years (range, 5–14 years). Statistically significant improvements in the mean Harris hip score were observed between the preoperative and final follow-up evaluations (62.71 ± 13.85 vs. 84.03 ± 16.21, respectively). The major orthopaedic complication rate was 16.5% (26/157). Six (3.8%) patients underwent revision at a mean of 3.5 ± 1.3 years after conversion, predominantly because of prosthesis loosening or recurrent dislocation. Nine (5.7%) patients developed prosthesis loosening at a mean of 2.6 ± 1.1 years following conversion, two of whom requested revision surgery. Eleven (7.0%) patients developed prosthesis dislocation, four of whom requested revision surgery. Conclusion CTHA may yield favourable functional outcomes and a reduced rate of major orthopaedic complications.

Background: Instability is a common complication following total hip arthroplasty (THA). The dual mobility cup (DMC) allows a reduction in the dislocation rate. The goal of this systematic review was to clarify the different uses and outcomes according to the indications of the cemented DMC (C-DMC). Methods: A systematic review was performed using the keywords “Cemented Dual Mobility Cup” or “Cemented Tripolar Cup” without a publication year limit. Of the 465 studies identified, only 56 were eligible for the study. Results: The overall number of C-DMC was 3452 in 3426 patients. The mean follow-up was 45.9 months (range 12–98.4). In most of the cases (74.5%) C-DMC was used in a revision setting. In 57.5% DMC was cemented directly into the bone, in 39.6% into an acetabular reinforcement and in 3.2% into a pre-existing cup. The overall dislocation rate was 2.9%. The most frequent postoperative complications were periprosthetic infections (2%); aseptic loosening (1.1%) and mechanical failure (0.5%). The overall revision rate was 4.4%. The average survival rate of C-DMC at the last follow-up was 93.5%. Conclusions: C-DMC represents an effective treatment option to limit the risk of dislocations and complications for both primary and revision surgery. C-DMC has good clinical outcomes and a low complication rate.

Background: Total Hip Arthroplasty (THA) for femoral neck fractures in the elderly is still lauded as one of the most successful surgeries of the century with good short and medium term outcomes. Postoperative dislocation remains the mainset back of hip replacement in the elderly. This study mainly sees keto assess early outcomes of total hiparthroplasty using dual mobility acetabular cup for femoral neckfractures in the elderly with emphasis ondislocation. Patients and Methods:We carried out 80 consecutive total hip replacements using a double articulating acetabular component. We assessed dislocation rates after 6 months and cumulative risks in the years following replacement.Results: After a mean follow-up of 50 months, only 8 (10%) deaths were recorded. We observed 1 case of dislocation at 4 months postoperatively that required revision arthroplasty. No case of instability was recorded. Conclusion:Dual mobility acetabular cups remain suitable implants in the face of postoperative dislocation after total hip replacement for femoral neck fractures in the elderly.
