Total plasma fibronectin as a marker of pregnancy-induced hypertensive disorders: A longitudinal study*1

Abstract: The findings of the present study confirm those of previous studies that have found increased total plasma fibronectin levels in pregnancy-induced hypertensive disorders. This study discovered that in these women, total plasma fibronectin levels are elevated in the first trimester. Total plasma fibronectin appears to be a poor predictor of preeclampsia when measured in a general pregnant population. Therefore, total plasma fibronectin should not be used as a routine screening test in a low-risk population. How… Show more

“…In rats, short term alcohol leads to accumulation of fibronectin in the heart (Vendemiale et al, 2001). In pregnant women, increased plasma fibronectin levels is utilized as a marker for gestational hypertensive disorders (Paarlberg et al, 1998) and fetal fibronectin is associated with the onset of labor (Cunze et al, 1996). Finally, these data may also have physiologic implications on uterine endothelial cell shape and function (Grinnell et al, 1982).…”

“…22,23 Such a test would enable identification of a high-risk group that may benefit from close monitoring, steroid administration, and anti-hypertensive therapy. VEGF antagonists such as sflt-1 may counteract the endothelial effects of VEGF and thus prevent or ameliorate the disease and allow progression of the pregnancy.…”

“…One study measured fibronectin at 13 weeks, but the authors only performed a trend analysis based on 3 consecutive measurements in the first, second and third trimester [16] . Our data indicate that former PS patients -previously identified with a subnormal pre-pregnant PV -may already have raised fibronectin levels at 12 weeks.…”
