Total Sesamoidectomy for Painful Hallux Rigidus: A Medium-Term Outcome Study

Abstract: High levels of clinical improvement and patient satisfaction were found following total sesamoidectomy. No deleterious consequences of sesamoid removal were observed. For symptomatic patients where a joint replacement/fusion is not indicated, total sesamoidectomy was beneficial as an interim procedure, for joints with a moderate (grades 2 to 3) degree of arthrosis.

“…29 Whilst this is less problematic for podiatric surgery, [30][31][32] it is much more taxing for podiatric therapies involving 'biomechanical' interventions, where good evidence is lacking. 29 Whilst this is less problematic for podiatric surgery, [30][31][32] it is much more taxing for podiatric therapies involving 'biomechanical' interventions, where good evidence is lacking.…”

“…11 If the first metatarsal is of normal length, shortening by osteotomy can introduce a risk of transfer metatarsalgia or stress fracture. 24 A recent systemic review 21 on distal metatarsal periarticular osteotomy reported that 30.5% of patients developed postoperative metatarsalgia. In the present study, preoperative examinations revealed that 8 (19%) of the 42 cases had metatarsalgia under the second or third metatarsal head.…”

“…Operative treatments for symptomatic hallux rigidus include cheilectomy, 4,10,16 distal metatarsal osteotomy, 2,15-17,29 phalangeal osteotomy, 5,25 sesamoidectomy, 24 interpositional arthroplasty, 13,23 arthroscopic debridement, 14 implant arthroplasty, 1,26 and arthrodesis. 6,9 Cheilectomy has been recommended as a first-step procedure for early-stage hallux rigidus with intact articular cartilage, 4,16 and is usually combined with other operative methods such as osteotomy or arthroplasty.…”

“…However, total sesamoidectomy for painful hallux rigidus has been reported to not cause such complications. 22 It is unlikely that the sesamoid itself but rather the surrounding ligamentous structures that are insufficient in causing the deformities present in surgical excision of the sesamoid, and possibly why it has not been reported in any cases of sesamoid agenesis, either partial or complete, until now.…”
