Total subaxial reconstruction

Abstract: Object. Certain cervical spinal conditions require decompression and reconstruction of the entire subaxial cervical spine. There are limited data concerning the clinical details and outcomes of patients treated in this manner. The object of this study was to describe the specific technique employed to perform a total subaxial reconstruction and review the postoperative outcomes following surgery.Methods. The author performed a review of data prospectively collected in 27 consecutive patients undergoing complet… Show more

“…Significant multilevel cervical spinal deformities secondary to disease processes such as rheumatoid arthritis may require anterior, posterior, or combined approaches. 31 In these cases, the use of interfacet spacers may be useful to increase foraminal area, facilitate decompression of the cervical nerve root, and decrease the risk of iatrogenic foraminal stenosis. Interfacet allograft can be used in combination with laminectomy or laminoplasty for management of spinal cord compression.…”

“…The contribution of facet resection to postlaminectomy instability has been well documented [2,14,15,17,29]. Thus, it is important that patients should not be considered for posterior procedures if their preoperative sagittal alignment is not lordotic, and they cannot be corrected with traction and reduction [29,38,44]. The principles of anterior column lengthening, posterior column shortening, and hinge correction around the posterior longitudinal ligament are recommended [3,10,11,29,33,35,36].…”

“…Present-day cervical spine instrumentation allows for anterior vertebral lengthening and fixation as well as dorsal rigid stabilization [38]. This manuscript provides an insight to results prior to the new instrumentation with long-term follow-up.…”

“…In such cases, the PAP approach with an initial posterior release, anterior correction and reconstruction, and posterior fixation with or without further correction was used. 1,24 Case examples are provided in Figures 3 and 4. The choice of anterior approach (anterior cervical diskectomy and fusion, anterior osteotomy, anterior cervical corpectomy and fusion, or hybrid construct) was determined by the individual surgeon.…”
