Total transvaginal endoscopic abdominal wall hernia repair: a NOTES survival study

Abstract: This novel approach seems technically challenging but feasible using equipment and accessories currently available for conventional laparoscopic and interventional endoscopy with low intra-abdominal contamination and sepsis. New procedure-specific instruments and equipment need to be developed to allow the surgeon safer access and more degrees of instrument freedom.

“…Last year, Wood published a small case series of six pure transvaginal ventral hernia repair procedures performed between November 2010 and February 2012 [7]. Others publications are known, but these describe transvaginal hernia repair in animals [8][9][10][11]. The IEHS even designated a chapter to the new technological developments in part 3 of the recently published Guidelines for laparoscopic treatment of ventral and incisional abdominal wall hernias [12].…”

“…Using principally animal models, numerous operations were described, including bowel anastomoses, solid organ resection, hernia repairs, and diagnostic explorations. [19][20][21][22][23][24][25] Transgastric access…”

“…They are most frequently caused by perforating fixation devices. Implementating the natural orifice transluminal endoscopic surgery (NOTES) technique in hernia repair could further reduce the number and severity of adverse effects [4][5][6][7][8]. The main benefit of the NOTES technique is the preservation of the integrity of the abdominal wall [9][10][11][12].…”

“…The feasibility and advantages of different access routes to perform intra-peritoneal onlay mesh (IPOM) repair in ventral wall hernias using the NOTES technique has been demonstrated in experimental studies [5,7,8]. The technical aspects concerning the transfer and fixation of mesh as well as the choice and final closure of different access routes are still controversially discussed [4][5][6][7][8]13]. In the present study, we assumed that fibrin sealant (FS) mesh fixation delivered by transgastric access (TGA) was beneficial in the attempt to minimize surgical invasiveness.…”
