Totally laparoscopic pancreaticoduodenectomy for locally advanced gastric cancer

Abstract: Although TLPD for locally advanced gastric cancer is a technically difficult challenging operation that requires careful dissection along the major vessels, intracorporeal tie sutures, and the placement of an external drainage tube into a narrow pancreatic duct, this procedure is technically feasible and safe in the hands of experienced surgeons. Long-term follow-up is mandatory to validate oncological outcome.

“…Laparoscopic dissection of the posterior pancreatic margin is particularly challenging. As optimal exposure is key, placing the camera port on the right side seems to be more conducive than having it at the umbilicus. The use of modern energy devices facilitates this step, but it is not entirely clear how safe it is in sealing the fragile pancreaticoduodenal arteries.…”

“…Technically-demanding procedures such as total gastrectomy with or without splenectomy, gastrectomy with multivisceral resection, and completion total gastrectomy for remnant gastric cancer have been successfully performed by experienced surgeons using minimally invasive approaches 64,65,66,67,68. Single incision or reduced port surgery for gastric cancer also has been reported to be a safe and feasible procedure 26,27.…”

“…For a long time, the laparoscopic approach to pancreatic surgery was thought to be only indicated for tumour‐staging purposes. Due to the fact that many problems have not yet been solved, especially related to anastomotic techniques and a high number of related complications, laparoscopic pancreaticoduodenectomy is still not established . In contrast, an increasing number of LDP over the past few years have shown technical feasibility .…”
