Toward a valid definition of gout flare: Results of consensus exercises using delphi methodology and cognitive mapping

Abstract: Objective. To identify, in people known to have gout, the testable, key components of a standard definition of gout flare for use in clinical research. Methods. Consensus methodology was used to identify key elements of a gout flare. Two Delphi exercises were conducted among different groups of rheumatologists. A cognitive mapping technique among 9 gout experts with hierarchical cluster analysis provided a framework to guide the panel discussion, which identified the final set of items that should be tested em… Show more

“…While a consensus approach has been initiated to establish a definition of a gout flare that can be uniformly applied in clinical trials and clinical practice (37), such a definition has yet to be finalized. The definition used in the main analyses of the current study included items recommended in the consensus statement (37) and was based on the patient-reported clinical characteristics that have been suggested to increase specificity for a gout flare (43).…”

“…The definition used in the main analyses of the current study included items recommended in the consensus statement (37) and was based on the patient-reported clinical characteristics that have been suggested to increase specificity for a gout flare (43). When another definition of gout flare used in a prior study was applied, the relative changes were similar to those of the main analysis, representing significant reductions of gout flares with rilonacept.…”

“…However, these clinical surrogates have not accurately identified gout flares, and potentially underestimate or overestimate the rates of gout flares. The lack of a standard definition of gout flare in observational studies demonstrates the intrinsic difficulty of flare identification (8). Therefore, better methods need to be investigated to identify gout flares.…”

“…Gout‐related variables included symptoms duration, classification according to the 1977 preliminary American College of Rheumatology (ACR) criteria 12, disease‐related comorbidities, previous and current treatment for gout, swollen and tender joint counts on 66 of 68 joints, measurement of tophi (count and tape measure of all the clinically evaluable tophi) 13, 0–10 on a visual analog scale (VAS) for pain, patient's global assessment of disease activity (PtGA), general health and physician's assessment of response to treatment (PhGA), flare occurrence (patient‐reported acute and significant worsening of joint pain) 14, and sUA levels. All patients completed the Italian versions of the Health Assessment Questionnaire (HAQ), the Short‐Form 36 (SF‐36) health survey, and the Gout Impact Scale (GIS) 15, 16, 17.…”
