Toward an Effective Strategy for the Diffusion and Use of Clinical Information Systems

Abstract: The full impact of IT in health care has not been realized because of the failure to recognize that (1) the path from availability of applications to the anticipated benefits passes through a series of steps; and (2) progress can be stopped at any one of those steps. As a result, strategies for diffusion, adoption, and use have been incomplete and have produced disappointing results. In this paper, we present a comprehensive framework for identifying factors that affect the spread, use, and effects of IT in th… Show more

“…Practice management systems comprise tools to help medical practices set appointments, track patients, identify charges, track payments, and manage accounts receivable. 4 A CIS, on the other hand, is a collection of tools that contain patient-carespecific data, such as outpatient services, inpatient physician orders and treatment, nursing interventions, and laboratory and radiology results. Although the terms electronic health record and electronic medical record are used interchangeably in the literature, the term electronic medical record will be used because the focus of this article describes how measurement of computer literacy for CIS users is an imperative strategy for the ultimate adoption of the (local) EMR.…”

“…The full impact of IT resources in healthcare has not yet been fully realized because of the failure to recognize the importance of cooperative alliances in the healthcare industry. It is critical for small healthcare centers to develop and implement a truly effective strategy to achieve the benefits of IT resources throughout the healthcare system via cooperative alliance (Davidson and Heineke, 2007). Therefore, efficiency in the use of existing IT resources among alliance members is of great importance for the survival of these small healthcare centers.…”

“…Whatever the benefits for the healthcare industry as a whole, investments in IT resources do not always result in more patients or higher payments for adopters and thus do not always reward them with higher revenues (Davidson and Heineke, 2007). Investments in IT resources are a long term process.…”

“…1,2 Repeated measurement of the change prior to, during, and after implementation has been reported as a strong method to obtain an overall review of this complex technology adoption process. 3,4 Evaluation of an information technology can be based on different indicators, [5][6][7][8] and a multimethod evaluation combining both quantitative and qualitative approaches has been suggested as a means of obtaining different perspectives to provide a deeper understanding of technology use. 9 The purpose of this study was to assess the overall impact of technology by using different approaches to evaluate the responses of nurses who have used a nursing information system (NIS) for 2 years.…”
