Towards fast online intrafraction replanning for free-breathing stereotactic body radiation therapy with the MR-linac

Abstract: The hybrid MRI-radiotherapy machines, like the MR-linac (Elekta AB, Stockholm, Sweden) installed at the UMC Utrecht (Utrecht, The Netherlands), will be able to provide real-time patient imaging during treatment. In order to take advantage of the system's capabilities and enable online adaptive treatments, a new generation of software should be developed, ranging from motion estimation to treatment plan adaptation. In this work we present a proof of principle adaptive pipeline designed for high precision stereo… Show more

“…Of all available imaging modalities for IGRT, MRI is the most versatile and suitable candidate as it provides soft-tissue contrast to enable direct tumour visualization as well as OAR localization (Lagendijk et al 2014). Moreover, it provides real-time imaging to characterize and eventually track anatomical motion (Stemkens et al 2016, Dietz et al 2017 for MRI guided radiotherapy and for dose reconstruction (Glitzner et al 2015) and ultimately real-time plan adaptation (Kontaxis et al 2017).…”

“…Existing pretreatment tools for quality assurance (QA) in the MR‐linac are typically time‐consuming solutions which, besides, are not applicable for an online adaptive workflow . Alternative patient‐specific QA solutions have been proposed, such as fast sanity checks on the adapted plan, in vivo geometrical accuracy of the delivery using EPID images, or the use of independent calculations fed with linac log files …”

“…[25][26][27][28] Alternative patient-specific QA solutions have been proposed, such as fast sanity checks on the adapted plan, 29 in vivo geometrical accuracy of the delivery using EPID images, 30 or the use of independent calculations fed with linac log files. [31][32][33][34] An MR-only workflow would allow for MRI-based delineation while performing dose calculation on a synthetic computed tomography (CT) derived from that MRI study. The use of log files in combination with an independent dose calculation algorithm using the synthetic CT is an alternative treatment verification method.…”

“…Thus, the previously considered ideas for ad hoc plan adaptation are particularly interesting for the next generation of linear accelerators, the MR‐linac. Various fluence‐based concepts for adaptive planning have been developed . On the other side, the current state of MR‐linac technology does not yet offer dynamic arcs and thus excludes VMAT.…”
