Lung transplantation in Japan is an increasingly accessible treatment option for end-stage lung disease; however, the lack of donor organs is a persisting challenge. Five- and 10-year survival rates of lung transplant recipients in Japan are comparable, if not superior, to international standards. The outcomes of lung transplantation in Japan are likely affected by multiple factors. Infectious disease complications are a significant burden to transplant recipients and account for approximately 30% of recipient mortality in Japan, presenting a major challenge in peri-transplant management. Herein, we explore the current status of infectious disease epidemiology, available evidence surrounding infectious diseases in lung transplantation, and potentially influential factors pertinent to lung transplantation outcomes in Japan. Although infection remains the major cause of morbidity and mortality associated with lung transplantation in Japan, there is limited data and evidence. Despite some uncertainties, publicly available data suggests a low rate of antimicrobial resistance in Gram-negative bacteria and a distinct set of endemic pathogens that recipients may encounter. As a countermeasure against the burden of infectious diseases, 8 out of 10 transplant centers in Japan have a dedicated infectious diseases department. Despite these efforts, specific surveillance, prevention, and management are indispensable to improving post-transplantation infectious disease management. We accordingly lay out potential areas for improving infectious disease-related outcomes among lung transplant recipients in Japan.

Introduction: Toxoplasma gondii is an intracellular protozoan commonly found in humans in the form of three main forms: tachyzoite, tissue cyst and oocyst. The definitive host is the Felidae family, and one of the intermediate hosts is humans. The most common routes of infection are ingestion of raw or undercooked meat, water and food contaminated with oocysts. Among the forms of infection, we distinguish between the acquired form and the much more dangerous congenital form. Aim of the study: The objective of the study was to rewiev the literature describing the dangerous impact of Toxoplasma gondii infection on human.Methods and materials: : We reviewed the literature available in the PubMed database, using the key words: "toxoplasma gondii"; "congenital toxoplasmosis"; "pregnacy".Results: T. gondii infection is particularly dangerous in pregnant women, due to the high risk of vertical transmission of the infection to the developing fetus. The clinical presentation of congenital toxoplasmosis depends on the gestational age at which the infection occurred. Pathognomonic signs of congenital toxoplasmosis include Sabin-Pinkerton Triad, which includes inflammation of the retina and choroid of the eye, dilatation of the brain's ventricular system and intracranial calcifications. Available pharmacotherapy works on tachyzoites present in the initial phase of acquired infection, congenital infection and reactivated infection in immunodeficient patients, available drugs do not work on tissue cysts.Conclusion: The consequences of Toxoplasma gondii infection are particularly dangerous for the fetus, so pregnant women should pay special attention to hand hygiene and thorough washing of the food they eat. For this reason, it is extremely important to educate pregnant women about health issues and draw attention to the possible consequences of T.gondii infection.

This study aimed at evaluating the potential of Copaifera lucens, specifically its oleoresin (CLO), extract (CECL), and the compound ent-polyalthic acid (PA), in combating caries and toxoplasmosis, while also assessing its toxicity. The study involved multiple assessments, including determining the minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC) against cariogenic bacteria. CLO and PA exhibited MIC and MBC values ranging from 25 to 50 μg/mL, whereas CECL showed values equal to or exceeding 400 μg/mL. PA also displayed antibiofilm activity with minimum inhibitory concentration of biofilm (MICB50) values spanning from 62.5 to 1000 μg/mL. Moreover, PA effectively hindered the intracellular proliferation of Toxoplasma gondii at 64 μg/mL, even after 24 h without treatment. Toxicological evaluations included in vitro tests on V79 cells, where concentrations ranged from 78.1 to 1250 μg/mL of PA reduced colony formation. Additionally, using the Caenorhabditis elegans model, the lethal concentration (LC50) of PA was determined as 1000 μg/mL after 48 h of incubation. Notably, no significant differences in micronucleus induction and the NDI were observed in cultures treated with 10, 20, or 40 μg/mL of CLO. These findings underscore the safety profile of CLO and PA, highlighting their potential as alternative treatments for caries and toxoplasmosis.
