Trabecular Bone Score: A Noninvasive Analytical Method Based Upon the DXA Image

Abstract: The trabecular bone score (TBS) is a gray-level textural metric that can be extracted from the two-dimensional lumbar spine dualenergy X-ray absorptiometry (DXA) image. TBS is related to bone microarchitecture and provides skeletal information that is not captured from the standard bone mineral density (BMD) measurement. Based on experimental variograms of the projected DXA image, TBS has the potential to discern differences between DXA scans that show similar BMD measurements. An elevated TBS value correlates… Show more

“…The DXA scans of vertebrae L1-L4 obtained to assess lumbar spine BMD can also be used to generate TBS, thereby producing complementary skeletal information from the same examination [6]. TBS has been shown to be significantly associated with direct measurements of bone microarchitecture and mechanical behavior [5,[8][9][10] and is predictive of current [6,[11][12][13][14][15][16] and future [17][18][19] fragility fractures in postmenopausal women with primary osteoporosis [19,20]. TBS can also be used to assess fracture risk in patients with specific causes of secondary osteoporosis such as hyperparathyroidism, diabetes, and glucocorticoid treatment [21,22].…”

“…A low TBS value is associated with fewer, less well-connected and more widely distributed trabeculae, while high TBS va lues are correlated with better trabecular structure (11). TBS can be readily applied to a DXA image through the use of a specific software, and has an attractive feature which is the possibility of being retrospectively calculated from an existing DXA image without the need for further imaging (12).…”

“…We also measured change in trabecular bone score (TBS), an indirect measure of lumbar spine bone microarchitecture and independent predictor of fracture risk. 7 TBS was determined for L1-L4: TBS ⩾ 1.350 normal; 1.200-1.350 partially degraded; o1.200 degraded trabecular microarchitecture. 7 Throughout the transplant period, patients were prescribed 1200 mg elemental calcium, and 2000 IU vitamin D daily.…”
