Trajectories of functioning in a population-based sample of veterans: contributions of moral injury, PTSD, and depression

Abstract: Background Although research has shown that exposure to potentially traumatic and morally injurious events is associated with psychological symptoms among veterans, knowledge regarding functioning impacts remains limited. Methods A population-based sample of post-9/11 veterans completed measures of intimate relationship, health, and work functioning at approximately 9, 15, 21, and 27 months after leaving service. “…We found that a considerable majority of participants (75%) were resilient, with stable low levels of anxiety over time. Our findings are consistent with prior studies that have documented resilience as the most common pattern in response to a wide variety of stressors, including school shootings (Mancini, Littleton, & Grills, 2016), combat (Maguen et al, 2020), traumatic injury (deRoon-Cassini, Mancini, Rusch, & Bonanno, 2010) and bereavement (Mancini et al, 2011). The findings are also consistent with a longitudinal cohort study of 1296 participants in Australia in which similar anxiety trajectory patterns emerged, including a resilient pattern characterizing 77% of the sample.…”

“…PMIEs most reliably predict assignment to trajectories characterizing poor or declining functioning [26]. It may be that individuals who do something to violate their moral code (e.g., directly or by failing to stop others from engaging in immoral acts) have more difficulty reengaging their values after this violation, leading to poorer functioning, than those who are betrayed.…”
