Trajectory and predictors of quality of life in first episode psychotic mania

Abstract: Results suggest that QoL can be maintained early in BD, and reinforce the importance of assertively treating depressive symptoms throughout the course of this disorder. The emergence of a link between premorbid adjustment and poorer QoL in this cohort highlights the importance of assessing facets of adjustment when planning psychological interventions.

“…Quality of life is reduced in both symptomatic and non‐symptomatic patients when compared to healthy controls,15, 16, 17 and several domains of functioning have been identified by patients as being of particular importance‐ including physical, sleep, mood, cognition, leisure, social, spirituality, finances, household, self‐esteem, independence, identity, work, and education 18. For both psychosocial functioning and quality of life, impairments are more pronounced in patients with depressive symptoms,19, 20, 21 in those with more previous episodes/longer duration of illness,20, 22 and in those with lower cognition 23…”

“…9 Patients with bipolar disorder live about half their life in a symptomatic state, primarily one of depressive symptoms, which may carry a risk of interfering with continuous employment. 10,11 In patients with depressive symptoms, impairment of psychosocial function is more severe, [12][13][14] and often prevents them from maintaining the ability to function at work. 15 Patients with bipolar disorder have also been found to have functional impairment between episodes.…”

“…However, cross-sectional studies cannot illuminate directionality of effects. Limited investigation of the trajectory of QoL following first episode psychotic mania did not reveal an effect of symptoms at baseline with QoL at 18 months (Oldis et al 2016): more sensitive analyses of time-varying relationships in the general BD population is needed to illuminate possible relationships. Additionally, although QoL change is typically assumed to be a downstream consequence of symptom changes in BD, it is not known whether changes in QoL may have reciprocal impacts on BD symptoms: potentially, improvements in wellbeing and functioning could moderate symptoms of the disorder (Murray & Michalak, 2012).…”
