Transanal advancement flap repair of transsphincteric fistulas

Abstract: The results of transanal advancement flap repair in patients with no or only one previous attempt at repair are good. In patients who have undergone two or more previous attempts at repair the outcome is less favorable. Remarkably, the number of previous attempts did not adversely affect continence status.

“…For example, cutting seton has a recurrence rate of 0%-28%, 12,13 and an incontinence rate of 20%-77%, 12,14 while the 'sphincter sparing' advancement flap technique has a recurrence rate of between 25% and 65% and an incontinence rate of 30%. 15 The Surgisis Biodesign Fistula Plug (Cook Medical, Bloomington, IN, USA) is associated with recurrence rates of between 17% and 86% and an incontinence rate of approaching zero, 16,17 while the ligation of fistula track procedure has yielded recurrence rates of 5%-60% and a incontinence rate of 6%. 18 The data in this study were collected prospectively.…”

“…The mucosa advancement flap (MAF), recently considered the best available treatment option for complex fistulas, results in a success rate of about 60% [5,6] . This procedure is technically demanding and may affect continence [7] . Use of fibrin glue is an alternative to the MAF; however, the long-term closure rates are low [8][9][10][11][12] .…”

“…Various techniques have been described including fistulotomy [1][2][3][4], fistulectomy [5] and advancement flap procedures [6][7][8][9][10][11]. The results of fibrin glue treatment have been disappointing and the technique has largely been abandoned [12].…”
