Transcatheter closure of a patent ductus arteriosus in a preterm infant with an Amplatzer Vascular Plug IV device

Abstract: Rapid advances in paediatric interventional cardiology allow transcatheter closure of patent ductus arteriosus (PDA) in increasingly smaller patients. We describe a successful transcatheter closure of a large tubular PDA in a symptomatic preterm infant of 2,210 g using an Amplatzer Vascular Plug IV device.

“…Zahn et al 18 implanted the Amplatzer Vascular Plug II in six patients and Philip  et al 19 closed the PDA with this device in older premature infants. Prsa and Ewert selected the Amplatzer Vascular Plug IV,20 but the length of these devices is not adapted to a short tubular DA. The Amplatzer duct occluder II21 has been successfully implanted but the retention discs are too large for infants weighing <1500 g. The new ADOIIAS, designed to close the PDA of premature infants,13 has the suitable properties and profile to close a tubular DA.…”

“…Depending on the size and shape of PDA, various types of devices have been used in transcatheter closure of PDA. For most children and adult with PDA, safe and effective transcatheter closure of PDA has been achieved with ADO2,5,14,15). Also, varying sizes of PDA had been occluded with the use of single or multiple coils3,16).…”

“…However, ADO II was not available in South Korea at that moment. After various types of AVP have become available, a few cases of successful PDA closure using these devices, even in preterm neonates, have been reported5,15).…”

“…Dennoch ist bei älteren, klinisch stabilen Frühgeborenen der interventionelle PDA-Verschluss auch bei einem Körpergewicht unter 3 kg mög-lich[26]. sen werden kann allein der ASD vom Sekundumtyp (ASD II), der mit 80% allerdings auch den größten Anteil aller Vorhofdefekte bildet.Der interventionelle Verschluss ist auch hier die Therapie der Wahl.…”
