Transcatheter implantation of the biological Sapien Edwards valve in the pulmonary position – first experiences

Abstract: A b s t r a c tWe present the first in Poland, and among the first in Europe, cases of transcatheter implantation of the biological Sapien Edwards valve in the pulmonary position. The valves were implanted successfully without periprocedural complications in 2 patients with postoperative right ventricular outflow tract dysfunction.

“…The SAPIEN TM valve is available in sizes larger than the current Melody ® , and therefore may be offered to patients with larger right ventricular outflow tracts (RVOT). We have recently reported on a single case of a successful pulmonary Edwards SAPIEN ™ valve implantation in a patient after RVOT correction with a patch . Now, we report a successful percutaneous Edwards SAPIEN ™ valve implantation in the pulmonary position in nine patients with severe pulmonary regurgitation after previous surgical repair with a RVOT patch.…”

“…Interestingly, a case series of four such patients successfully treated with PPVI using the Melody ® valve (Medtronic, Minneapolis, MN) have recently been described . Furthermore, successful implantations of the Edwards SAPIEN ™ transcatheter heart valves, which are commonly used but in the aortic position, have recently been reported in pulmonary conduits . The SAPIEN TM valve is available in sizes larger than the current Melody ® , and therefore may be offered to patients with larger right ventricular outflow tracts (RVOT).…”

“…SAPIEN valves are bovine pericardial tissue valves that are hand sutured to stainless-steel radiopaque stents, available in sizes 23, 26, and 29 mm. Tey are used after CE certifcation in the pulmonary position and have been previously reported in adult patients [34][35][36][37][38]. Te delivery system is more complex and designed for aortic valve implantation, resulting in signifcant difculty when negotiating the RVOT because of the curves involved, especially in smaller hearts.…”

“…SAPIEN3, equipped with an outer pericardial skirt, is designed to minimize perivalvular leak. The valve, manually crimped with a specialized tool, is implanted using the Retroflex-3 or Novaflex delivery system (Edwards Lifesciences) consisting of a guiding catheter and a single-balloon catheter [19–22]. …”
