Transcranial Doppler to detect on admission patients at risk for neurological deterioration following mild and moderate brain trauma

Abstract: Increased pulsatility index after mild or moderate TBI is a reason for concern about the possibility of further neurological deterioration. Computed tomography and Doppler measurements could be combined to detect on admission patients at risk for secondary neurological deterioration in order to improve their initial disposition.

“…Recientes estudios han demostrado el papel pronóstico de la sonografía intracerebral en los enfermos traumáticos basándose en la velocidad e índice de pulsatilidad en las primeras horas tras el TCE 11,15 . Un reciente estudio preliminar 12 mediante DTCC mostró que determinados parámetros hemodinámicos se correlacionaban con un peor pronóstico en los enfermos con TCE.…”

“…Brain oedema increases cerebral circulation resistance. First, the diastolic blood flow velocity decreases [8] and the pulsatility index then starts to increase [9][10][11]. The diastolic blood flow velocity decreases more than the systolic flow, which directly leads to an increase in the PI value [7,12,13].…”

“…In a prospective cohort study ( n = 78) set in the emergency room of a French district hospital, consecutive patients admitted with mild or moderate TBI underwent both CT and transcranial Doppler (TCD) studies within 12 hours of admission [8]. Patients were then assessed, based on objective predefined criteria, for neurological deterioration 7 days after admission.…”
