Transcystic Choledochoscopy Utilizing a Disposable Choledochoscope: How We Do It

Abstract: Background: Biliary disease is common occurrence and can make up a large portion of the practice of a general surgeon. Choledocholithasis is a common entity amongst those with biliary disease. Although modern trends favor endoscopic retrograde cholangiopancreatography (ERCP) and other imaging modalities for the diagnosis and management of choledocholithiasis, laparoscopic common bile duct exploration (LCBDE) is likely underutilized. Methods: A literature summary utilizing a PUBMED search was performed to provid… Show more

“…As for laparoscopic and choledochoscopic gallbladder-preserving cholecystolithotomy, choledochoscope is applied based on laparoscopic cholecystolithotomy, which helps improve the operative visual field and complete the stone extraction under direct vision. It not only reduces the difficulty of stone extraction, shortens the operation time, and facilitates postoperative recovery, but also avoids unnecessary injury of the gallbladder due to operation errors, causes less microstructure damage of the gallbladder, and contributes to postoperative recovery of gallbladder function [ 13 15 ]. The results of this study showed that the operation time was shortened and the intraoperative blood loss was reduced in the laparoscopic + choledochoscopic group.…”

“…In addition, transcystic choledochoscopy remains an important tool in the treatment of common bile duct (CBD) stones in patients who have previously undergone a Roux-en-Y gastric bypass. 21 There are, however, both complications and limitations associated with choledochoscopy. Overall, the rate of adverse events (AEs) for peroral cholangioscopy is estimated to be between 2% and 30%.…”

“…LC and LCBDE: This approach has all the benefits of a minimally invasive operation and ensures the resolution of concomitant gallbladder and CBD stones in a single session, as does traditional open CBD exploration[ 34 ], thus avoiding any complications of preoperative ERCP-ES (pancreatitis, cholangitis, bleeding, duodenal perforation)[ 35 , 36 ]. However, it requires specific equipment and advanced training that encourage the vast majority of surgeons to prefer the two-stage procedure by preoperative ERCP-ES[ 5 ].…”
